Permanency Coordinator

Who are we looking for?
The ideal candidate is passionate about working in human services and has a bachelor’s degree in social work, counseling, or a related field. We are looking for a coordinator that can provide all of the SWAN service units, including but not limited to Child Profiles, Child Preparation Services, Child Specific Recruitment, Family Profiles, Placement, and Finalization. We provide services in the home and community settings.  We are looking for individuals who demonstrate commitment to diversity, equity, and inclusion and engage in ongoing self-exploration and growth. Every Child is looking for an energetic and passionate permanency coordinator to work closely with referral sources to develop a comprehensive permanency plan in collaboration with clients and their families. This position is primarily responsible for identifying adoptive/kinship families and connecting them with additional support (i.e. completing an assessment to identify individualized family service, providing in-house visits, continued support, and case advocacy) to ensure stable permanent placements. This is a full-time 40-hour per-week position.

Requirements: Bachelor’s degree in social work, counseling, or related field; two years related experience; experience working with trauma issues; excellent organizational skills with good computer skills; willingness to work a flexible schedule, which may include off-business hours, such as after school, evening, and/or on weekends; ability to adapt to change; a sense of humor; strong ability to personally relate one-on-one with each client and build solid relationships.
