Summary
The Senior Care Advocate is a key position responsible for driving business growth by establishing and maintaining referral relationships within the healthcare and senior services industry. Reporting to the Owner, this individual will focus on marketing Always Best Care's In-Home Care and Senior Living Placement services to healthcare providers, including case managers, social workers, discharge planners, and senior living facilities. The Senior Care Advocate will be a proactive presence in the local community, attending networking events and establishing partnerships with healthcare organizations to generate referrals.
This is a field-based role, with an equal focus on In-Home Care and Senior Living Placement referral activities. The territory includes central Phoenix, Ahwatukee, Tempe and neighboring areas. Therefore, having reliable transportation is a must. The ideal candidate will be an articulate, trustworthy professional that is an excellent communicator who is at ease presenting our services to both professional audiences and families in need of care.
Job Functions & Responsibilities
Outreach, Relationships, and Customer Service:
  • Build and nurture long-lasting relationships with healthcare providers such as hospitals, skilled nursing facilities, rehab centers, home health agencies, and senior centers to generate referrals for In-Home Care and Senior Living Placement.
  • Identify, research, and pre-qualify potential referral sources and accounts.
  • Meet with professional referral sources/decision makers in person, with a target of 40-50 contacts per week, to promote Always Best Care services and demonstrate value.
  • Follow up on leads and prospective client inquiries in a timely manner, conducting initial intake calls, scheduling and performing assessments for prospective clients.
  • Create and deliver personalized Care Plan and assist clients in exploring options for Senior Living Placements, including tours of senior communities and assisted living residential homes.
  • Connect prospective clients with additional resources to help them achieve their goals.

Community-Based Outside Sales Efforts:
  • Represent Always Best Care at senior-focused networking events, community gatherings, and professional meetings.
  • Conduct presentations to educate healthcare professionals and community groups on aging comfortably and safely.
  • Participate in health fairs, awareness days, and other relevant community events to promote the Always Best Care brand.

Activity Targets and Tracking:
  • Maintain accurate records of all referral sources and sales activities in the company's CRM system.
  • Provide weekly sales reports to the Owner/Director of Operations, discussing progress, challenges, successes, and new business opportunities.
  • Participate in regular sales meetings to review performance against targets, set new goals, and strategize for continued agency growth.
  • Ensure sales activities align with the marketing budget and meet overall departmental goals.

Teamwork:
  • Work closely with the Operations and Care Management teams to ensure smooth transitions for new clients.
  • Collaborate on sales and marketing strategies to strengthen community outreach and build strategic partnerships.
  • Demonstrate a collaborative and team-oriented approach, participating in ongoing operational and procedural improvements.
  • Respond respectfully and in a timely manner to all staff, clients, referral partners and community members.
