Job description

A-Line Staffing is now hiring Fully Remote Social Workers in California. This would be a 4-month long contract opportunity with a Fortune 500 healthcare company based in California. This would be full time / 40+ hours per week.

Social Worker Compensation

  • The pay range for this position is $28.00 - $30.00 per hour based on experience and credentials


Social Worker Schedule

  • Schedule is Monday – Friday 8:00am-5:00pm PST


Social Worker Position Summary:

Effective 1/1/27, Health Net, CalViva and CHPIV will discontinue the ALF Transition community support service.  The discontinuation will require that we support the transition of our vulnerable members from ALF setting to alternate setting, including home, SNF/LTC, RCFE/ARF.  Team members will carry a caseload of 300+ members they will support through these transitions.  The team will be lead by LTSS Social Workers and the PHCO leadership team.


Social Worker Responsibilities

·       This is a Remote Phone-based position - Call ALWs to follow up on member transitions and member needs

·       Speak to Skilled Nursing Facilities/LTC providers, Members and their families about transitions


Social Worker Requirements

·       MUST RESIDE IN CALIFORNIA FOR CONSIDERATION

·       3+ years of Skilled Nursing Facility, Assisted Living Facility,

·       Prior Social Worker, Case Manager, or Care transition planning experience

·       Familiarity with Medicare, Medicaid, Medi-Cal health plans and regulatory documentation

·       Excellent computer skills with MS Office applications

·       Comfortable being on the phone all day and speaking with providers, members, and health professionals
