Social Services Assistant (Part-Time)

Location: Hadley, Massachusetts, United States

Job Summary:

At Hadley Pointe for Nursing Rehab and Care, we are dedicated to providing exceptional care to our residents while fostering a warm and supportive community. We are seeking a compassionate and organized Social Services Assistant to join our team. As a part-time member of our team, you will play a vital role in ensuring the social, emotional, and psychological well-being of our residents.

Responsibilities:

  • Provide emotional support and companionship to residents, engaging in activities such as conversation, games, and outings

  • Assist with daily living activities, including personal care and ADLs, under the supervision of licensed staff

  • Collaborate with the social services team to develop and implement individualized care plans

  • Facilitate resident activities, events, and group programs

  • Maintain accurate records and documentation, including resident assessments and service provision

  • Participate in team meetings and contribute to a positive and supportive work environment

Requirements:

  • High school diploma or equivalent required

  • Previous experience in a healthcare or social services setting preferred

  • Ability to work varying shifts, including weekends and holidays
