Job Overview
Ebenezer is looking for an Adult Day Program Assistant to join our team in Burnsville, MN This role is responsible to encourage socialization and assists in planning and implementation of active programming needs of individual senior day center participants and child care groups. Responsible for ongoing charting, case management, and assisting with personal care duties.

Ebenezer Ridges Care Center highlights a full continuum of care campus including Transitional Care, Adult Day Programs, Intergenerational Child Care, Long Term Care, Enhanced Care and Assisted Living/Memory Care. We are located across the street from Fairview Ridges Hospital at the I-35E and I-35W split in Burnsville, MN. Responsibilities:
  • Ability to provide direct care for the Adult Day Program participants.
    • Responsible for preparation, implementation and cleanup of daily programs i.e. crafts, woodworking, games, music, etc.
    • Perform routine personal care and grooming at the center (including baths).
    • Responsible for assisting identified participants with all toileting needs.
    • Communicate with staff the participants needs and individual plan of care.
    • Report any problems, progress, and observations to the Director.
  • Assist the Director of Adult Day with participants' individual case management.
    • Maintain monthly charting, quarterly care plans and health updates.
  • Maintain a clean and safe environment for participates.
    • Maintain safe area for participants.
    • Maintain adequate supplies for cleaning and caring for participants.

Required
  • Current driver's license in good standing
  • 0 to 1 year of previous experience

Preferred
  • CNA experience preferred but will consider direct care experience in lieu of certification.
  • Windows/Microsoft previous experience
  • Customer Service
  • Interact and communicate empathetically and effectively with tenants/clients, families, etc.
  • Interpersonal skills. Assist with ADL's
