Project IMPACT is partnering with Clarity HR Solutions to find qualified and passionate Part-time Case Managers to serve children and families in their Family Preservation program. We are looking for experienced Case Managers bilingual in English and Spanish with previous experience working in a Family Preservations Program.

About the Job

Case Manager provides case management duties to assigned clients, which include weekly in-office/field-based case management, providing community resources/linkages, ensuring the safety/security of children by observation, etc.


Specific Job Duties

  • Responsible for providing in-office and field-based case management services to assigned clients weekly or as required.
  • Work with a Clinical Supervisor Therapist to assist in the identification of community linkages to ensure that families are provided with full range of available resources.
  • Observe family environment to ensure safety/security of children in the home.
  • May conduct initial assessment of family prior to services starting and/or case management services assigned.
  • Develop individualized service plans based on assessment reports provided.
  • Evaluate parent/child/family progress and provide progress letters to the Clincial Supervisor.
  • Communicate observed behavior, progress and/or problems with Program Manager, and/or Clinical Supervisor.
  • Assist with preparation of reports, correspondence, and processes statistical data.
  • Document weekly visits and submit documentation to quality assurance staff as required.
  • Submit weekly reports to Clinical Supervisor for review.
  • Promote all Project Impact Programs and Services to clients and community.
  • Maintain confidentiality of client, employee, and management related duties.
  • Assist in other duties as required.


Job Requirements

  • Minimum Bachelor’s Degree in Social Work, Child Counseling, Development, Psychology or Human Services.
  • Must have 2 years of case management experience minimum.
  • Must have 2 years of Family Preservation documentation writing experience.
  • Must be bilingual in English and Spanish.
  • Previous experience case management experience in a mental health or social service agency setting preferred.
  • Proficiency in MS Word, Excel, and a computer database.
  • Ability to interact and communicate both verbally and written with seriously emotionally disturbed children, adolescents, resource parents, caretakers, school personnel, physicians, county agencies, other healthcare providers, etc.
  • Excellent client services, verbal and written communication skills; must have the ability to multi-task, meet deadlines, and work with all levels of management.
  • Must have reliable transportation to travel while providing case management services to assigned clients, provide and maintain current personal vehicle insurance coverage.
  • Organized, detail oriented, the ability to work in a fast-paced environment while following through on work assignments.
