Schedule:Monday-Friday, 40 hours/week; schedule is between the hours of 8 a.m. to 6:30 p.m. Flexible schedule of 4x10s or 5x8s

What You'll Do: In this high-impact role, you'll help patients stay on track with preventive and chronic care while keeping the care team organized, responsive, and proactive. Panel Management 

  • Maintains chronic disease registry; enters and updates information from patient charts accurately, completely and timely 
  • Proactively performs pre-visit activities for patient visits in collaboration with Care Teams 
  • Collaborates with team to ensure appropriate tracking and follow-up of referrals 
  • Utilizes electronic reports, registries, and review of provider schedules to proactively assess needs for prevention screening and communicates via Electronic Health Record (EHR) 
  • Provides patient education as appropriate 

Quality Improvement/Quality Assurance 

  • Helps develop integrated care pathways advancing the delivery of efficient and comprehensive care 
  • Understands metric specifications and internal workflows around metrics and clinic metric goals 
  • Acts as a resource to medical assistants on a day-to-day basis to answer questions, review, and explain documentation, workflows and other metric details 
  • Participates in ongoing process improvement projects and supports the clinic in meeting metric goals as needed 

Medical Assistant Functions (as needed basis) 

  • Manages the full patient experience onsite, documenting patient history and vitals and providing follow-up instructions and help with referrals 
  • Administers immunizations, injections, and medications 
  • Performs blood draws

Who You Are: You're organized, proactive, and energized by team-based care. You're ready to jump in, stay on top of details, and help patients get the care they need. In this role, you'll bring: 

  • Current MA certification from a nationally recognized program (such as AAMA) 
  • Minimum 2 years of Medical Assistant 
  • Current BLS/CPR certification 
  • Ability to work effectively as part of a team 
  • Excellent organizational skills and ability to prioritize work independently 
  • Exceptional interpersonal and communication skills (face-to-face, via email/writing, and on phone)
  • Strong attention to detail
  • Strong critical thinking and problem-solving
  • Familiarity with computers and Microsoft Office, and/or ability to learn use of new programs/software
  • Ability to complete a pre-employment background check specific to the position applied for

Preferred: 

  • Bilingual in Spanish 
  • Prior experience in community health clinic 
  • Experience in primary care setting 
  • Familiarity with OCHIN Epic 
  • Prior professional or volunteer work with communities lacking access to healthcare due to poverty, language barrier, housing instability, mental illness, etc.
