Job description

Responsibilities:

  • Designs and facilitates the Community Memory Care program, promoting active participation from residents, family members, and team members
  • Oversees the budget and implementation of a cost-effective Memory Care program, including managing an inventory list of equipment and supplies and maximizing resources available
  • Creates and sustains a welcoming and safe environment which promotes resident wellness and independence
  • Participates in the resident move-in process through initial wellness assessments and the development of individualized care plans
  • Trains, schedules, and leads the Memory Care clinical team in providing exceptional care experiences, providing hands-on support, education and training opportunities
  • Delivers health promotion services and dementia care education services for residents and families
  • Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience
  • Contributes toward quality assurance and performance improvement initiatives
  • Coordinates the Community's infection control program while maintaining state regulatory compliance

Qualifications:

  • Maintains current state Registered Nurse (RN) licensure, Social Work licensure, or Dementia Care Professional certification
  • Bachelor's degree preferred
  • Minimum of three (3) years experience in dementia care
  • Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team members
  • Excellent written and verbal communication skills
  • Demonstrates exceptional interpersonal skills and ability to serve with love, compassion, and dignity
