Job description

WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. We serve the most vulnerable seniors with better quality and compassion in a value-based model, and the Program Manager plays a vital role by directing nutritional services for our participants in the WelbeHealth day center.

The Program Manager focuses on overseeing all aspects of meal service and supervising the kitchen aide team members. Reporting to the Program Director, the Program Manager is a member of the WelbeHealth management team, also spearheading engagement and personal care services.

Essential Job Duties:

  • Accountable for the delivery of high-quality nutrition, kitchen, reception, engagement, and personal care services to ensure participants are uniquely cared for
  • Maintain high standards of participant satisfaction and service recovery, partnering with center leadership to address concerns, resolve issues, and foster a welcoming, participant-centered environment.
  • In coordination with the Engagement Coordinator and Day Center Supervisor, manage nutrition, engagement, personal care, housekeeping, and front desk reception operations, including hiring, training, monitoring, and mentoring staff and ensuring clear and effective workflows
  • Collaborate with Program Director to plan and facilitate effective interdisciplinary team (IDT) and other participant-care related meetings, ensuring they are organized and engaging, and that follow-up items are documented and tracked
  • Coordinate and assist with WelbeHealth physical plant, supplies and equipment management, which includes monitoring and coordinating all ongoing maintenance and vendor oversight, as well as ensuring a safe, clean, and welcoming physical environment for participants, staff, and visitors
  • Oversee the engagement department assessments and interventions to assure regulatory compliance and timely documentation

Job Requirements:

  • Bachelor's degree in relevant field; five (5) years of experience may be substituted in lieu of education and master's degree in relevant field a preferred
  • Minimum of one (1) year of relevant management experience; ADHC/PACE/Health Plan/Clinic operations a plus with demonstrated experience working with elderly living at home
  • Two (2) years of supervisory experience within a senior or health care program preferred
  • Professional experience in a relevant field such as Nursing, Social Work, Psychology, or Occupational or Physical Therapy preferred
  • Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
