About the role
The Hospice Manager coordinates the referral, admission and care delivery for hospice patients, caregivers and families. The Hospice Manager develops, reviews, and modifies the patient care plan as discussed in IDG. Plans, organizes, and conducts discharge planning specific to referral of patients for hospice services through interaction with personnel in hospitals, health agencies, physicians, and staff in physicians' offices. Reviews referrals for medical necessity, appropriateness of service and complete plan of care. Provides direct supervision for Hospice RNs, Hospice Aides, LPNs, Hospice Liaisons and Nursing Assistants. Is responsible for staff development and quality assurance. Participates in all aspects of the management of the service. In the absence of the Director, may assume responsibility for the agency.
Essential Job Functions
  • Assists in taking referrals by telephone from physicians, facilities or the community and provides information concerning hospice services
  • Understands fiscal reimbursement systems for patient services, applies this knowledge to the assessment of incoming referrals, and communicates same to health professionals, physicians and patients/families for referral.
  • Assists in discussing and receiving Certification of Terminal Illness and other physician orders.
  • Demonstrates clinical expertise by applying current knowledge and research in hospice clinical practice.
  • Reviews and evaluates care being given, providing instruction, guidance, and counseling to staff delivering care, including field or facility supervision.
  • Provides for direction in coordination of interdisciplinary services.
  • Develops and maintains staffing and scheduling practices that enhance patient care and allow staff satisfaction; utilizes on call, call-in, and overtime as appropriate to ensure patient safety.
  • Reviews and evaluates care being given, providing instructions, guidance, and counseling to staff delivering the care.
  • Plans in advance to address staffing variances; responsible for maintaining a sufficient number of qualified staff to meet patient care needs. Including developing contingency plans for potential staffing shortages as well as low visit periods.
  • Plans, develops, implements educational plan and competency program for all levels of clinical staff
  • Understands fiscal reimbursement systems for patient services, applies this knowledge to the assessment of incoming referrals, and communicates same to health professionals, physicians, patients and/or families.
  • Acts as a liaison between patients, families and members of the health care team to resolve practice issues.
  • Provides for direction in coordination of multidisciplinary services including assisting team with rehabilitation services for patients.

Requirements
Educational Requirements: Bachelor's and master's degrees in nursing, healthcare administration, public health or related field. Registered Nurse currently licensed by the State of Hawaii.
Experience: Minimum of five (5) years of progressive work in a nursing environment, preferably hospice, oncology or acute care. Minimum of two (2) years experience in a hospice agency is preferred. Previous supervisory experience is required, preferably in a hospice agency.
Skills Required:
  • Working knowledge of computer applications, including electronic medical records.
  • Valid driver's license for the State of Hawaii and current insurance coverage
  • Good driving record, as evidenced by driver's current abstract.
  • Current CPR certification.
