Home Hospice Case Manager

Job Title: Home Hospice Case Manager

Profession: Registered Nurse

Experience: Minimum experience in home hospice case management required.

License: Valid RN License required.

Certifications: BLS required.

Must-Have: Experience with home hospice case management is mandatory. Census for this role includes approximately 35-45 patients. EPIC charting system experience is required.

Description: The Home Hospice Case Manager is responsible for providing quality care to patients in a home setting. Experience in home hospice case management is essential for this position. Candidates must have a valid RN license and BLS certification. This role entails managing a census of 35-45 patients. Familiarity with the EPIC charting system is required. Candidates should bring a supportive and compassionate approach to patient care. This position may require flexibility and the ability to float based on facility needs.
