Hospice Clinical Liaison / Business Development Representative
Next Care Hospice is seeking an experienced, relationship-driven Hospice Clinical Liaison to join our growing team.
We are looking for someone who understands hospice, knows how to build trust with referral partners, and is passionate about helping patients and families navigate end-of-life care with dignity and compassion.
This is not an entry-level marketing role. We are seeking a professional who is confident in hospital, SNF, assisted living, physician, and community-based relationship development and who can serve as a strong clinical and operational resource to referral partners.
The ideal candidate is proactive, highly organized, relationship-focused, and experienced in hospice admissions, referral management, and healthcare networking.
Responsibilities
  • Develop and maintain strong referral relationships with hospitals, skilled nursing facilities, physicians, case managers, discharge planners, assisted living communities, and healthcare partners
  • Educate healthcare professionals, patients, and families on hospice eligibility, services, and benefits
  • Serve as a trusted resource throughout the referral and admission process
  • Coordinate closely with Intake, Clinical, and Operations teams to ensure timely and seamless admissions
  • Participate in community outreach, networking events, facility in-services, and strategic growth initiatives
  • Maintain accurate referral activity, follow-up, and relationship management documentation within CRM/EMR systems
  • Identify growth opportunities within assigned territories and provide market feedback to leadership
  • Support census growth initiatives while maintaining a patient-centered approach to care
Qualifications
  • Minimum 3 years of hospice or healthcare marketing/business development experience required
  • Existing relationships within hospitals, SNFs, physician groups, assisted living, or healthcare systems preferred
  • Strong understanding of hospice eligibility, admissions processes, and end-of-life care services
  • Excellent communication, presentation, and relationship-building skills
  • Ability to work independently while collaborating effectively with interdisciplinary teams
  • Experience with EMR/CRM systems preferred
