- Serve as a trusted resource throughout the referral and admission process
- Coordinate closely with Intake, Clinical, and Operations teams to ensure timely and seamless admissions
- Participate in community outreach, networking events, facility in-services, and strategic growth initiatives
- Maintain accurate referral activity, follow-up, and relationship management documentation within CRM/EMR systems
- Identify growth opportunities within assigned territories and provide market feedback to leadership
- Support census growth initiatives while maintaining a patient-centered approach to care
Qualifications
- Minimum 3 years of hospice or healthcare marketing/business development experience required
- Existing relationships within hospitals, SNFs, physician groups, assisted living, or healthcare systems preferred
- Strong understanding of hospice eligibility, admissions processes, and end-of-life care services
- Excellent communication, presentation, and relationship-building skills
- Ability to work independently while collaborating effectively with interdisciplinary teams
- Experience with EMR/CRM systems preferred
- Professional, compassionate, and service-oriented demeanor

- The opportunity to make a meaningful impact in the lives of patients and families
At Next Care Hospice, we believe hospice is more than a service โ it is a commitment to dignity, compassion, and quality of life. We are looking for team members who share that mission and want to grow with an organization committed to excellence in patient care.
