À propos du mandat :

Services SFT est à la recherche d’un(e) Travailleur(se) social(e) pour Forestville au sein du réseau de la santé sur la magnifique Côte Nord. Vous aurez l’occasion de travailler auprès d’une clientèle variée dans un cadre stimulant.

Responsabilités principales :

  • Évaluer la situation psychosociale des usagers et de leur famille.
  • Intervenir auprès de clientèles en difficulté (santé mentale, dépendance, jeunesse, personnes âgées, etc.).
  • Élaborer et mettre en œuvre des plans d’intervention individualisés.
  • Assurer un accompagnement et un soutien aux usagers dans leur parcours de soins.

Exigences :

  • Baccalauréat en travail social ou en service social.
  • Membre en règle de l’Ordre des travailleurs sociaux et des thérapeutes conjugaux et familiaux du Québec (OTSTCFQ).
  • Expérience récente et pertinente en dépendance
  • Formation suicide / homicide, IGT et autres outils d'évaluation en dépendance
  • Véhicule requis;
  • Formation évaluation régime de protection doit être faite ou la personne la personne s'engage à la faire dès le début de l'entrée en poste.
  • Excellente capacité d’écoute, d’adaptation et de travail en équipe.
  • Disponibilité à travailler à temps complet pour toute la durée du mandat.
