Your Role

As a Nurse Practitioner, you'll manage your own panel providing comprehensive care across the lifespan - diagnosing, treating, prescribing, and leading team huddles that close preventive and chronic-care gaps. You'll also engage patients in shared decision-making, support transitions of care, and contribute to our ongoing Quality Assurance and Performance Improvement (QAPI) program.


Requirements

  • Licensed APRN (Family or Adult-Geriatric NP) in Vermont with prescriptive authority.
  • Minimum 1 year of post-licensure experience in family practice or primary care.
  • Commitment to patient-centered, trauma-informed, high-quality care.
  • Proficiency with EHRs (we use Medent).

Cold Hollow Family Practice is seeking a dedicated and skilled Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide comprehensive care to patients across all age groups, focusing on preventive care, health education, and management of acute and chronic conditions. This role requires a strong clinical background, excellent communication skills, and the ability to work collaboratively within a multidisciplinary team!


Responsibilities

  • Conduct thorough patient assessments, including physical examinations and medical history evaluations.
  • Develop and implement individualized care plans based on patient needs.
  • Provide acute and chronic disease management, including medication administration and monitoring.
  • Perform diagnostic evaluations and interpret results to guide treatment decisions.
  • Administer various treatments such as IV infusions, suturing, and botulinum toxin treatments.
  • Manage patient care
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Maintain accurate medical documentation in compliance with HIPAA regulations.
  • Educate patients and families on health management, disease prevention, and wellness strategies.
  • Participate in clinical research initiatives as needed.

Qualifications

  • Current certification as a Family Nurse Practitioner with an active state license.
  • Experience in primary care or family medicine is preferred; additional experience in specialties such as pediatrics, geriatrics, or internal medicine is a plus.
  • Proficient in EMR/EHR systems preferably Medent.
  • Familiarity with CPT coding, ICD-9/ICD-10 coding systems is essential for accurate medical documentation.
  • Strong clinical skills in areas such as triage, acute pain management, behavioral health, and emergency medicine.
  • Knowledge of infectious disease care and experience working in critical care settings (ICU or Level I/II trauma centers) is advantageous.
  • Ability to provide compassionate care to diverse populations including those with developmental disabilities or chronic illnesses.
  • Strong organizational skills with the ability to manage multiple priorities effectively.
