The Enrollment Specialist provides outreach services to hard-to-reach populations. They are responsible for locating individuals who need of health home services and enrolling them into the program. This includes outreach to HARP eligible and enrolled members, educate patients about the HARP and Home and Community Based Services (HCBS) benefit package, and assist with the HARP enrollment process. The Enrollment Specialist will provide assistance, when necessary, to ensure patients have active Medicaid. The Enrollment Specialist is responsible for assisting care coordinators with the following outcomes: Reduce utilization associated with avoidable and preventable inpatient stays; reduce utilization associated with avoidable emergency room visits; improve outcomes for person with mental health illness and/or substance use disorders; and improve disease-related care for chronic conditions. They must be available to work flexible hours during the day, evening and/or weekend. The Enrollment Specialist reports directly to the Director of Engagement and Enrollment, who shall determine the specific responsibilities of the position. These responsibilities may be revised, as necessary, with proper notification and training provided, where appropriate.

Essentials Functions:

  • Delivers outreach services to high risk populations who need health home services.
  • Provides educational information to potential members in the form of brochures and other program literature that supports health home services.
  • Demonstrates developing knowledge of community resources.
  • Assists in the completion and review of all opening paperwork, e.g.: Patient Consent Form (DOH 5055), Patient Bill of Rights, Member Information Sheet, Emergency Coverage Policy, HIPAA Notice of Privacy Practices, Text Consent Form, and the Code of Conduct.
  • Works with local emergency departments to identify, screen and enroll new health home patients.
  • Assists the patient in identifying any immediate needs.
  • Assists the patient with obtaining active NYS Medicaid, as necessary.
  • Makes all initial linkages and advocates with service providers based upon the immediate needs care plan, (eg: medical provider, Department of Social Services).
  • Conducts street-level outreach to targeted communities in order to identify and engage potential new members.
  • Represents Health Home care coordination at community events, (e.g.: health fairs, community outreach events, local charity events).
  • Documents all patient specific outreach efforts in Health Home care coordination software systems as well as in the “OES Excel Tracking File”.
  • Utilizes local RHIO (Regional Health Information Organization/HEALTHeLINK™) on an ongoing basis to identify new demographic information on potential patients.
  • Coordinates with community referral sources and/or managed care organizations to improve outreach efforts and successfully engage referred patients.
  • Assists the Care Coordinator with linkage and referrals to needed services to support care plan/treatment goals, including medical/behavioral healthcare, patient education and self-help/recovery and self-management.
  • Assists the Care Coordinator with advocating for services and assisting with scheduling of needed services.
  • Monitors/supports/accompanies the patient to scheduled medical appointments, as needed.
  • Accountable for engaging patients in care.
  • Attends initial orientation and training, required annual trainings, and other trainings to strengthen skills and improve program outcomes.
  • Adheres to policies regarding working hours, break periods and proper use of the Association’s ADP payroll and benefits system.

    • Qualified candidate will have a High School diploma and two (2) years of qualifying experience. Candidate must demonstrate experience in care coordination with the following populations: persons with a chronic illness and/or person with a history of mental illness, homelessness, or chemical dependence. Experience with families preferred.
    • Required Skills
      • Must possess valid NYS driver license, insurance and dependable car to use for patient services activities including transporting patients, when necessary.
      • Ability to teach and influence others.
      • Demonstrated ability to work effectively in a team environment.
      • Effective interpersonal relationship and customer service skills.
      • Must be available to work flexible hours including days, evenings and/or weekends. Must be available to work a minimum of one (1) out of three (3) evening or weekend shifts
      • Fluent in reading, writing, and speaking English. Some positions require fluency in English, Spanish and/or additional languages.

      Required education: Associate degree in Health and Human Services and one (1) year of qualifying experience
