Job Description

Community & Veterans Programs Coordinator

Trellis Supportive Care is seeking a compassionate and mission driven Community & Veterans Programs Coordinator to develop and implement community education and Veterans programming that promote awareness of Trellis Supportive Care services within the 13-county service area.

Qualifications:

  • Bachelor’s degree in human services field or equivalent in experience and training
  • Veteran status preferred
  • Familiarity with healthcare, hospice & palliative care services, Advance Care Planning, and/or the local Veteran Community preferred
  • Understands and committed to the Hospice philosophy of care
  • Understands and maintains working knowledge of community resources, Veteran resources, and Advanced Care Planning guidelines
  • Public speaking and presentation skills

Responsibilities:

  • Develop and maintain relationships with key partners and referrals such as Veterans constituents and local & regional officials in coordination with the Director of Community Programming
  • Meet regularly with Community Partnership councils and Veterans
  • Facilitate and plans Veteran’s programs and initiatives that align with Trellis Supportive Care’s mission
  • Participate in joint planning sessions with other agencies & organizations
  • Solicit opportunities from community organizations seeking speakers and presenters
  • Collaborate with other human service agencies to sponsor educational events
  • In coordination with the Director of Community Programming, plan and implement community education opportunities and events consistent with the mission and strategic goals of Trellis Supportive Care on such topics as: hospice and palliative care, Advanced Care Planning workshops and facilitator training, end-of-life care issues, programs for healthcare professionals, caregiver support programs and requested programming for faith communities
  • Provide information to constituents regarding the benefits of early admission to hospice care
  • Coordinate development and distribution of brochures and event flyers
