Shift: Monday to Friday

Essential Functions:

Duties & Responsibilities

· Monitors data quality and if needed, works with the Manager, Outcomes and Quality Improvement to develop and implement strategies and systems to improve data quality.

· Conducts data-related audits (including data system audits) and reports findings to program management and the Manager, Outcomes and Quality Improvement.

· Conducts user acceptance testing (UAT) to ensure proper functioning of data systems.

· Works with Manager, Outcomes and Quality Improvement and Program Managers to address deficiencies found in internal audits/monthly outcome and file reviews.

· Assists in development and facilitation of in-service trainings and meetings related to program performance, data quality monitoring, and contract compliance as well as data collection systems and outcomes analysis.

· Assists in the preparation for funder site visits specific to file maintenance, program performance, and completion of funder evaluation tools.

· Troubleshoots basic technical problems related to data entry and reporting within database systems used by the HART Initiative.

· Performs moderate to high volume of data entry into organization information systems.

· Performs other duties as assigned.

Education and/or Experience:

High School Diploma and seven years of related experience, or

Bachelors’ Degree and three years of relevant experience, or equivalent combination of education and experience.

Knowledge/Skills/Abilities (K/S/A):

Language Skills: Excellent oral and written communication skills. Strong interpersonal skills required. Demonstrated writing skills and competency in quantitative analysis may be required. Ability to provide clear oral and written technical directions.

Computer Skills: Advanced proficiency in Microsoft Office especially Teams, Word, Excel and Outlook, is required; experience with Salesforce, HMIS, Adobe Pro; and other software routinely used by Heartland Human Care Services a plus.

Work Arrangement:

This role is eligible for a “hybrid” or “remote” work arrangement. If hybrid, it is expected this person would be able to meet for “in person” meetings when needed or at the request of others. In-person meetings may be required on a regular basis (weekly, monthly, etc.). There may also be the availability to work from home when appropriate.

Work Environment:

· The noise level in the work environment is usually moderate.

· Program sites are located throughout the Chicago area, including the North and South sides of Chicago, as well as the suburbs. Employees may be required to report to any one of our locations on a temporary or permanent basis.

· The employee may be exposed to outside weather conditions.

Physical Demands:

While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel.

· This position demands that the employee be able to talk, hear and maintain close/distance vision.

· The employee is frequently required to reach with hands and arms.

· The employee is occasionally required to stand, walk and sit.

· The employee must occasionally lift and/or move up to 25 pounds.
