RN Community Liaison

Overview:

Commonwise Home Care is seeking a dedicated individual to join our team as a Nurse Educator. In this multifaceted role, you will be instrumental in building connections within the Williamsburg community, focusing on educating and supporting older adults and their families about aging, home care options, and resources available for aging in place or transitioning to senior living communities.

The Care Management aspect of this role also includes providing senior housing & relocation assistance, supportive consulting, family relationship management, healthcare crisis management, coordination of healthcare, home safety evaluations, and life care management to clients. They also serve as account managers for clients receiving home care services and nurse-aides providing their direct care.

This role truly is the best of both worlds for those passionate about the nursing field and networking in order to understand the needs of the aging adult community.

Guaranteed at least 10 hours a week. 

Key Responsibilities:

1. In-Home Care Assessments:
   • Conduct in-home assessments to evaluate healthcare needs of clients.
   • Collaborate with the interdisciplinary team to develop personalized care plans.
2. Care Management:
   1. Utilize nursing skills to provide expert care and support to clients in their homes.
   2. Maintain accurate and up-to-date client records 
3. Networking and Community Engagement:
   1. Leverage existing networks and build new connections in various community segments.
   2. Actively participate in local events and meetings to promote awareness of aging and home care options.
4. Education and Outreach
   1. Serve as a knowledgeable resource on aging and home care (through ongoing training with the marketing team) 
   2. Conduct educational sessions for individuals, families, and groups of older adults to inform and prepare them for future needs.
   3. Establish and maintain regular presentation schedules at local venues like churches, community centers, etc., to provide ongoing education and support.

Ideal Candidate Profile:

• Experienced RN 
  • At least 5 years of experience in the nursing field. 
• Educator Mindset:
  • Skilled at presenting information in an engaging and informative manner, catering to the needs of older adults and their families.
• Resourcefulness:
  • Knowledgeable about local resources and services related to aging, home care, and senior living options.
• Commitment to Service:
  • Passionate about making a positive impact in the lives of older adults and their families.
• Community Involvement:
  • Already networked and trusted within parts of the Virginia beach community.
  • Able to utilize personal connections to further the mission of Commonwise.
