Purpose:
The Transition Coordinator is a member of the interdisciplinary team providing pre and post transition services to persons with mental or physical illness who are part of the Williams or Colbert Class Membership. Some CM’s may also have a secondary diagnosis of substance abuse, intellectual and/or developmental disabilities or other clinical issues. Must be accessible via cellphone during work hours. Coordinator will work in Nursing Facilities and Community members homes to provide quality comprehensive services.
Essential Responsibilities, include but are not limited to:
- Transition Coordinator will provide services to assigned teams class members as a part of the interdisciplinary team.
- Transition Coordinator will be responsible for monitoring service plans for the class members assigned.
- Transition Coordinator will document CM’s level of functioning and update the care manager on care plan changes including: (1) functional status, (2) risk of harm, (3) medication status, (4) healthcare utilization, (5) psychosocial status, and (6) community engagement as needed, per program expectations.
- Assist with the coordination all the services each class member needs to achieve their community living goals.
- Transition Coordinator with assist the Interdisciplinary team with obtaining resources and providing referrals as needed.
- Work with Managed Care Organizations, to secure appropriate referrals, linkages, and appointments when requested by Interdisciplinary team.
- Facilitate and attend medical provider visits with Class Member’s as appropriate.
- Provide required visitation to class members transitioning to the community per program guidelines.
- Manage client transportation needs/issues, i.e., submitting approvals to First transit, Prime Care/Paratransit contact etc.
- Achieve required productivity targets and complete documentation in a timely manner, including clinical documentation and service reporting.
- In conjunction with the client’s treatment team, keep client’s family, guardian, and other Envision Unlimited staff informed of the client’s progress, including emergency room visits, hospitalizations, and medication changes/adjustments.
- Provide skill building to Class member when needed.
- Assist Class Members in obtaining all vital documents needed for transition.
- Coordinator will assist class members in the community with shopping for daily living essential as needed or identified for the care team.
- Provide and document crisis interventions to the Care Team as needed, within 24 hours of an incident.
- Other duties as assigned.

EDUCATION, SKILLS, and/or EXPERIENCE:
Bachelor’s Degree preferred in social work, counseling, psychology or related field with at least three years’ experience in social services, or 3 years’ experience working with clients in a mental health or health care setting required. Will consider non-bachelors degreed staff with more extensive case management experience and knowledge of services provided to Program participants.
