This position is eligible for employee benefits, including vacation/sick leave accrual, 10 holidays each year, health benefits, 401(k), and much more! Our office hours are Monday - Thursday, 8:00 am to 5:30 pm and our offices are closed on Friday - giving you a three-day weekend! The salary range for this position begins at $37,000 and goes to $46,000 annually.


Position Objective

This position is responsible for providing supportive service coordination to residents of two (2) senior living residential housing complexes. Saint Charles Place, located in St. Joseph, Missouri and St. Patrick’s Manor, located in Cameron, Missouri. Services provided are designed to be responsive to residents’ individual needs, utilizing the strengths-based model, and will link residents with services provided by either our own agency or our community collaborative partners.


Responsibilities

  • Conduct a comprehensive, non-clinical assessment of residents for wellness and social needs on a yearly basis.
  • Help residents to identify, access and coordinate services (such as personal care services), including monitoring of services provided and follow-up communication with service providers.
  • Monitor the receipt and follow through of services, including encouraging and motivating residents to engage with providers and participate in their own care/services management.
  • Proactively develop and arrange educational/preventative health programs and services for residents.
  • Develop and sustain partnerships with local disability resource centers, community-based supportive service providers and other community stakeholders.
  • Coordinate case management efforts with all staff and outside services to meet residents’ needs.
  • Maintain a file on each resident that includes assessment, personal goal plan, case notes documenting action taken, progress, challenges, and follow up. Files should also include all Agency required documentation (e.g., Bill of Rights, Release of Information).
  • Enter all required data into the agency database (ETO) per agency protocol.
  • Maintain comprehensive statistical data on all residents; submit required annual reports to HUD for appropriate properties and any other required funder and/or agency reports.
  • Maintain internal systems of outcome tracking.
  • Prepare case records for proper storage after resident exits program.
  • Submit expenditure report forms and vouchers monthly.
  • Work in conjunction with partners to determine eligibility and suitability of potential clients.
  • Stay abreast of current trends and resources available to residents.
  • Attend agency and community meetings as requested.
  • Attend in-service training and/or outside webinars, conferences, or workshops as required. HUD properties have a minimum of 11 hours per year, other properties have a minimum of 6 hours per year.
  • Perform other duties as assigned.
Requirements:


Qualifications

  • Bachelor’s degree in social work, human services, gerontology or psychology, from an accredited school and program is preferred; individuals without a degree but having appropriate work experience are also considered.
  • Knowledge of and training in elderly and disability services.
  • Knowledge of referral processes.
  • Demonstrate working knowledge of local services for the specific resident profiles and people with disabilities.
  • 2-3 years of social service delivery experience, preferably direct experience working with elderly people, or people with disabilities.

  • Ability to handle multiple demands and priorities and work with diverse population.
  • Conversant in and supportive of Catholic Charities’ mission.
  • Ability to consistently meet deadlines and produce timely and accurate data/reports.
