Intake Coordinator

BYSCM, an Aging Life Care Management company, is seeking an experienced human services professional to fill the role of Intake Coordinator/Marketing Liaison. This position is ideal for individuals with backgrounds in Psychology, Social Work, or Nursing who are dedicated to the care of the elderly, disabled, and cognitively impaired populations.

Primary Responsibilities:

• Conduct community outreach and educate on BYSCM services.
• Perform new client assessments/intake and handle onboarding and sign-on processes.
• Assist with overflow Care Management tasks as needed.
• Provide limited on-call coverage during weekends and evenings.
• Service areas include Milwaukee, Waukesha, Washington, and Ozaukee Counties.

Qualifications and Education Requirements:

• Active, unencumbered RN, LPN, MSW, or CSW license in the state of Wisconsin is preferred.
• Certification as a Certified Case Manager (CCM) or Care Manager Certified (CMC) is advantageous.
• Bachelor’s Degree in Psychology, Social Work, Nursing, or a related field is required.

Preferred Skills:

• Strong patient assessment skills and knowledge of healthcare systems.
• Proven experience in patient advocacy and understanding the needs of the frail elderly, disabled, or cognitively impaired.
• Experience performing home visits and familiarity with case management.
• Comprehensive knowledge of community resources.
• Ability to assess the cognitive, functional, and social needs of clients.
• Proficient in computer skills, including Outlook, MS Word, and Internet Explorer.
• Effective communication skills, assertiveness, and persuasiveness to advocate for client needs and facilitate service uptake.

This role demands a professional who can interact positively and creatively with clients and colleagues alike. The ideal candidate will have the ability to convincingly promote BYSCM services, complete necessary paperwork efficiently, and identify both immediate and long-term client needs.
