Job Description

ESSENTIAL JOB FUNCTIONS

Working under the direction of the Director of Nursing Services- Adult Division with clinical support from DDS; provide initial and on-going support and supervision to the Program RN’s and LPN’s; Responsible for the health, safety and wellbeing of developmentally challenged individuals residing or working within the CRI system. Tasks to include but not limited to:

  1. Maintains current working knowledge of CRI, DDS and ICF policies, mandate, protocols and standards in order to maintain best practices; ensuring compliance with same.

  1. Attends seminars and workshops to keep abreast of trends affecting medical care for the ID population; maintain all required training per agency policy and DDS regulatory requirement.

  1. In conjunction with the Director of Nursing Services (DNS) and the Associate Executive Director, maintains minimal licensing deficiencies in all programs, 100% active treatment in ICF/ID facilities to meet State and Federal regulatory requirements and ensures nursing best practices are utilized.

  1. Provides support and supervision to new and existing nurses. Functions as a resource person for the agency and provides support to Program RN’s LPNHC’s and LPN’s in determining approaches to take in relation to the health management of the clients.

  1. In conjunction with the Health Care Coordinator and the DNS, strives to ensure that all Program RN’s and LPN’s receive proper orientation and in-service training in respect to their positions.

  1. Ensures that evaluations of Program RN’s are completed in a timely manner.

  1. Maintains awareness of trends in errors and deficiencies and provides proper education to address these issues.

  1. Participates in all quality improvement projects and will keep administration abreast of all developments.

  1. Conducts audits and observations in all adult programs.

  1. Participates in committees as requested by the Associate Executive Director or the DNS-Adult Division.

  1. Serves as medical support for the staff in the group homes as necessary and in an on-call basis. In this capacity, familiarizes oneself with client’s general medical and behavioral needs and the effective utilization of emergency services at the local and state levels.

  1. Maintains confidentiality inside and outside the agency in all aspects of the client’s needs/problems and any issues that arise in any of the programs and adheres to CRI’s confidentiality policy.

  1. Maintains confidentiality inside and outside the agency regarding issues regarding any issues concerning another employee.

  1. Communicates with the DNS-Adult Division regarding any problems or concerns with another employee while maintaining confidentiality for all persons involved.

  1. Ensures all clients are provided with high quality services and care for health and safety.

  1. Ensures all nursing assessments and discharge summaries have all clients’ medical needs addressed and are submitted in a timely manner.

  1. Conducts quarterly physical assessments on residents, as needed.

  1. Responsible for the follow though of all medical needs for the clients by:
    1. Monitoring need and attendance of medical and dental appointments.
    2. Providing documentation of care in the form of DAR notes and quarterly assessments.
    3. Reviewing the medication administration record, in accordance with CRI policies as well as State and Federal regulations.
    4. Monitor staff weekly medication record audits.
    5. Delegates duties appropriately and provides ongoing support and education to unlicensed staff.
  2. Upon request, assists in the screening and interviewing of prospective nursing staff and makes recommendations to the DNS.

  1. Meets with the DNS for supervision, as needed.

  1. Create a nurturing, positive learning environment and develop strategies to help the clients cope effectively with social, emotional and physical situations of daily living and facilitate independent performance in these areas.

  1. Develops and implements action plans to rectify know problems.

  1. Evaluates action plan outcomes and makes adjustments to the plans, as needed.

  1. Conduct self in a professional manner when interacting with participants, supervisors, co-workers, families, medical profession and members of the community.

  1. Performs all other job duties as assigned.

EDUCATIONAL/PROFESSIONAL REQUIREMENTS

  1. Registered Nurse License for the State of Connecticut.
  2. Minimum of five years of experience as a Registered Nurse in the field of Intellectual Disabilities.

OTHER REQUIREMENTS

  1. Commitment to the Philosophy of community based services, integration and values of client centered services for the developmentally disabled client.
  2. Must possess a valid CT driver’s license, be able to drive own and agency vehicles and have a good driving record.
