Description:

The Intake Coordinator provides support to the Clinic team as the first contact for services for veterans and military-connected individuals in need of mental health care. He/she will manage all intake calls and appointments, administer appropriate paperwork and psychometric screeners, and will schedule with appropriate clinical staff for biopsychosocial assessments. The Intake Coordinator will work with a multi-disciplinary team for case assignments, consultations, and care coordination. The Intake Coordinator will receive cutting-edge training to support his/her clinical growth.

Summary of Responsibilities:

  • Greet and appropriately direct clients through the intake process while demonstrating excellent customer service.
  • Conduct and concurrently document detailed intakes in the electronic health record system within 1 business day of first contact with the clinic.
  • Maintain comprehensive client referral tracking system via review of data reports pulled weekly by the Data Manager
  • Evaluate the client’s need for services, make treatment recommendations, and appropriately match clients to clinicians based on areas of specialty and client’s presenting concerns.
  • Prioritize access to care by scheduling biopsychosocial appointments within a 2-week window of first contact.
  • Enter intake paperwork data into the electronic health record (EHR) within a timely manner.
  • Inform Lead Clinciian of high-risk cases for support and execution of appropriate safety protocols, while maintaining calm demeanor
  • Meet regularly with the Lead Clinician and clinic multi-disciplinary team to coordinate case assignments and report referral-to-admission conversion rates, screener completion rates, themes of client need, intake trends, as well as barriers to accessing care.
  • Conduct ongoing intake process evaluation to the Lead Clinician for continuous growth and quality improvement efforts.
  • Work collaboratively with the Outreach Manager to identify referral sources who need follow-up regarding appropriate referrals to the clinic.
  • Provide individual (adults and children), couples, family, and group therapy to clients, with a focus on delivering evidence-based practices within a targeted treatment model; up to 20% of workload will be providing therapy.
  • Perform and document client screenings, psychosocial assessments, and progress in treatment.
  • Engage with clinic’s data manager to foster a positive relationship around
  • data collection, trend analysis, research, and reporting.
  • Serve as a backup to the front desk as needed.
  • Perform other duties as assigned.
Requirements:

Qualifications:

  • Master’s Degree in psychology, social work and/or other mental health field.
  • Experience working with or connection to military/veteran population.
  • Seeking clinical licensure.
  • Knowledge of mental illness, mental health diagnoses and HIPPA; knowledge of various treatment modalities and methods; knowledge of community resources.
  • Professional written, verbal, and electronic communication skills.
  • Willingness to learn new functions and assist in establishing efficient processes.
  • Strong consultative abilities, conflict-resolution skills, and collaboration abilities.
  • Ability to work flexible hours to include 1-2 evenings per week.
  • Maintain a valid driver’s license and
  • Meet background check requirements.
  • Maintain a working cell phone.
