POSITION TITLE: CLIENT SERVICES COORDINATOR
DEPARTMENT: FAMILY SERVICES
REPORTS TO: Sr. CASE MANAGER
CLASSIFICATION: NON-EXEMPT, FULL-TIME
As a Client Services Coordinator working with an underserved population, you will be a key player in ensuring the seamless delivery of support services to individuals facing a myriad of problems. Reporting to the Sr. Case Manager you will coordinate and oversee the implementation of client-focused programs, working to enhance their access to vital resources and opportunities.
RESPONSIBILITIES:
- Conduct outreach as needed and intake assessments to identify clients' immediate needs and develop an understanding of their unique circumstances.
- Coordinate and facilitate the delivery of support services, including applying for SNAP, healthcare access, and mental health resources.
- Serve as a liaison between clients and external agencies, ensuring effective communication and collaboration to meet clients' comprehensive needs.
- Maintain accurate and up-to-date client records, documenting services provided and tracking progress.
- Implement and monitor client service plans, adjusting strategies as needed to address evolving needs and goals.
- Organize and lead workshops and support groups to enhance clients' life skills, employability, and overall well-being.
- Advocate for clients' rights and navigate social service systems to address legal, financial, and healthcare issues.
- Collaborate with community partners and stakeholders to expand the range of services available to clients.
- Conduct regular follow-ups with clients to assess progress and provide ongoing support.
- Administer and enter all program related data in adherence to contractual requirements.
QUALIFICATIONS:
- Associates Degree in Social Work, Psychology, Human Services, or related field preferred; relevant work experience may be considered.
- Proven experience working with homeless populations and understanding the challenges they face.
- Knowledge of community resources, social services, and support networks.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work collaboratively within a multidisciplinary team.
- Empathy, cultural sensitivity, and a commitment to promoting dignity and respect for all clients.
- Familiarity with trauma-informed care principles.
- Able to work flexible hours.
SKILLS:
- Case coordination and management expertise.
- Strong problem-solving and crisis intervention skills.
- Proficient in Microsoft Office Suite and client database management.
- Ability to work independently and efficiently in a fast-paced environment.
- Documentation and record-keeping skills.
