St. Gabriel's Hospice & Palliative Care is currently seeking a RN Case Manager to join our team in San Antonio!

Position Overview
Job Qualifications
Education: Must be a graduate of an accredited Nursing SchoolLicensure: Current state license as a Registered Nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license.
Experience: Two (2) years experience as a registered nurse in a clinical care setting. Hospice experience preferred.
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Must read, write and comprehend English.
Transportation: Reliable transportation and valid auto liability insurance.
Responsibilities
- Conduct initial assessments and ongoing evaluations of patients’ needs
- Develop and implement individualized care plans
- Collaborate with interdisciplinary teams to ensure holistic care
- Provide education and emotional support to patients and families
- Coordinate services, including home visits and referrals
- Maintain accurate documentation and records
- Participate in quality improvement initiatives
