What You'll Do as an RN Hospice Case Manager:
As an RN Hospice Case Manager, you will carry out our Mission of Love by assuring quality care of our Hospice Admissions in a caring, timely and appropriate manner, paying particular attention to management of patient's related symptoms and comfort level. Look for and share with leadership ideas for improvement in the organization.
Our RN Hospice Case Manager, is responsible for the following:
  • Maintains a working knowledge of standards of care for hospice programs in order to insure compliance for Medicare certification and state licensure of the hospice program. Completes documentation including assessments, certifications and care plans in accordance with the Medicare Conditions of Participation for hospice programs.
  • Attends and participates in IDG, mandatory staff meetings, and in-service programs.
  • Maintain communication and provide updates or changes in patient condition to the patient's family, primary care physician, facility staff and hospice medical director.
  • Monitors quality end-of-life care by routinely assessing patient's needs. Completes accurate documentation, and coordinates care with other members of IDG. Provide end-of-life education for patient, family members, caregivers and friends; responds to the physical, mental, social and emotional needs of terminally ill patient and their loved ones.
  • Revises/updates care plans and performs assessments of family/caregiver needs on an ongoing basis.
  • Pronounces death in the absence of a physician and makes all appropriate notifications. Complete all required documentation.
  • Assess, administer and document professional nursing care to assigned Hospice patients utilizing the nursing process in accordance with both the nursing and administrative policies.
  • Performs patient care responsibilities in an environment that optimizes patient/caregiver safety and reduces likelihood of medication/healthcare errors. Demonstrates knowledge of medications and their correct administration. Coordinates all patient services. Assesses pain and utilizes appropriate pain management. Offers support and palliative care.

Requirements for an RN Hospice Case Manager:
  • Current RN licensure by the Pennsylvania State Board of Nursing.
  • CPR certification prior to or upon employment.
  • Previous hospice experience.
  • Valid PA driver's license and insurance on personal vehicle.
  • Excellent mentorship program - we will train!
