Outreach and Risk Management Coordinator

POSITION SUMMARY:
Under the supervision of the Chief Operating Officer, the Outreach and Risk Management Coordinator is responsible for ensuring that patient risk management protocols and regulations are monitored continuously and that follow-up actions are taken, as necessary. Outreach and Risk Management Coordinator will coordinate various community outreach activities and will work to provide outreach opportunities for community residents who lack access to healthcare and health insurance, and build/strengthen enrollment collaborations with local partner organizations.
ESSENTIAL DUTIES, RESPONSIBILITIES, AND REQUIREMENTS:
Outreach (40%):
- Organizing community outreach events in churches, senior centers, and other community organizations, including identifying venues, and creating educational materials
- Distribute outreach materials to patients, community members, partner organizations and businesses.
- Develop, discover, and attend community events to promote the health center's services and the mission of the organization.
- Collaborate with various local organizations to build patients base, spur enrollment, and build referral linkages.
- Disseminate health center information at different community events, local businesses, community-based organizations, and other healthcare providers.
- Communicate and coordinate all community outreach activities with immediate supervisor.
- Input collected data into appropriate software for grant reporting.
- Provide education around sexual health, HIV/AIDS, and biomedical HIV prevention services
- Provides education, referral and linkage as needed to Post Exposure Prophylaxis (nPEP), Pre-exposure Prophylaxis (PrEP), treatment, and case management to community members.
- Represent SJH during outreach events with partners participating in tabling and providing health education and HIV testing as needed.
- Organize flu shot events at senior housing, daycares, schools, and other community partner locations.
Risk Management (60%):
- Assist Chief Operating Officer (COO) with monitoring the day-to-day operation of the organization's risk management program.
- Coordinates the risk management program to meet the needs of the department, grants and FTCA coverage.
- Prepare various incident summary reports; provides required documentation for investigations, activities and resolutions related to the reported incident(s).
- Assist the COO with executing an effective risk management communication program.
- Support the day-to-day risk management activities of Saint James Health
- Other Duties as assigned.
PREFERRED QUALIFICATIONS:
- Current New Jersey state license to practice as a Licensed Practical Nurse (LPN)
- Graduation from an approved Licensed Practical Nurse (LPN) educational program
- S. diploma or GED (or equivalent)
- Minimum of 1 year of risk management experience
- Good written and verbal communications skills
- Bilingual (preferred)
