Job Description:

This position will have dual reporting to the Chief Nursing Officer and the Client Incident Command. The primary objective of this position is to train, observe, and validate that staff caring for residents have a clear working knowledge of each resident’s care needs and the identified interventions by the Resident Care Team. To achieve this, it is essential that continuous and ongoing assessment occurs regarding staff knowledge. The CCBMs will perform the following tasks while also embedding the federal regulatory standards and the unique needs of each resident in their approach to work.

 

Essential Competencies, Duties, and Responsibilities

Daily tasks will include:

  • Participate in morning CCBM “check-in” with Client clinical leadership. This is an agenda-driven format and assures continuity in approach throughout the facility.
  • Lead and/or participate in unit-based “check-in” and facilitate conversations to include recent updates and follow-up with resident needs.
  • Make frequent daily rounds on residents prior to unit-based “check-in” and ongoing as needed daily. See the attached “Daily Triage Report” form.
  • Triage emerging issues that involve resident care issues. Identify priorities and guide care with nurse managers.
  • Look at the prior day’s falls, restraints, coaches, elopements, new wounds, changes in condition, behavior health issues emerging, physician orders, discharges and readmissions, and any other care concerns.
  • Review the EMR for the above issues identified and ensure documentation in the physician orders, care plans, Kardex, flow sheets, MAR, etc. is up to date and accurate.
  • Review with each staff person the current information in the resident’s Kardex and ensure staff is updated as needed.
  • Review with staff the current care plan for the residents on their daily assignment and the interventions that have been identified.
  • Provide training on how the care plan interventions are implemented daily.
  • Provide instruction on how to share feedback on the effectiveness of the care plan interventions.
  • Follow up with staff to update the care plan as issues emerge.
  • Observe, coach, and train staff on the transfer hand-off process between units.
  • Advise the resident care team during the resident care conference to ensure the care plan is updated with relevant interventions and documentation in the EMR. Follow-up changes to the care plan have been shared with frontline staff through the use of the Kardex tool.
  • Meet with residents weekly to learn if they have any concerns and evaluate if the care plan is reflecting resident concerns.
  • Communicate and coordinate resident follow-ups with the members of the Inter-disciplinary Team (IDT)
  • On a weekly basis, shadow one CNA and one licensed bedside nurse for 3-4 hours (individualized according to staff need) a week during their peak caregiving tasks (such as med pass). This will occur on different shifts.
  • Validate staff knowledge of the information shared using teach-back and/or other adult learning principles.
  • Provide weekly updates to Executive Leadership on all observations and evaluations of staff performance. Escalate urgent accountability issues as they occur.

 

Education and/or Experience

  • Ph.D. or Master’s level registered nurse or another medical degree professional or BSN with requisite experience.
  • Active license; not necessarily in CA.
  • At least 15 years of experience in Skilled Nursing Facility and management.
  • Knowledge and expertise in medical records and in creating a resident care plan.

 

Other Qualifications

  • Must be COVID vaccinated including at least 1 booster.
  • Must have Flu shot.
