Basic Function/Overall Responsibilities: Overall management of the agency intake and admissions process for OCFS residential and juvenile justice programs and school. Ensure timely and appropriate admissions/transfers to their departments in a manner that maximizes the utilization of all agency programs and best serves the needs of our clients and customers.

Principal Assigned Responsibilities:

  1. Review all referrals for assigned departments and work with Directors/Principal to make determinations around acceptances and admission of children into those programs.
  2. Maintain regular communication with referring sources regarding status of youth on waitlists.
  3. Facilitate the management of referrals between agency departments.
  4. Participate in weekly Foster Care rounds to gather information about referrals and provide timelines for transfers and admissions.
  5. Develop and monitor internal tracking mechanisms to anticipate openings. Work with Directors to identify youth who can be transferred to more appropriate levels of care within the agency.
  6. Participate in marketing and outreach efforts with Counties, school districts and partner agencies to maximize appropriate utilization of all agency programs.
  7. Maintain regular communication with HGS Diversion workers regarding placement needs of family court.
  8. Attend family court as needed regarding potential referrals.
  9. Participate in county Single Point of Accountability (SPOA) meetings, as needed, for our primary counties.
  10. Assess trends in referrals and referring agency needs and provide feedback for agency use in planning and program development.
  11. Will work directly with Program Assistants to manage tracking systems and ensure data in EHR referral module is accurate and up to date.
  12. Active participation in the agency's ongoing Q.I. activities and programs.
  13. Other related and/or agency-wide initiatives as assigned by supervisor.

Education and Experience Requirements:

  1. Bachelor’s degree in human services or other related field with three years of case management experience required.
  2. Knowledge of federal and state initiatives related to residential care highly preferred.
