Certified Community Health Worker CHW

A-Line Staffing is now hiring a Community Health Worker. The Community Health Worker would be working for a Major Healthcare Organization in San Jose, CA and has career growth potential. This would be full time / 40 hours per week.

Community Health Worker Compensation:

• The pay for this position is $21.38-$23.50 hourly
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Community Health Worker Highlights:

• The required availability for this position are as follows: 40 hours per week between Monday - Friday 8am – 7pm, with possible rotating Saturday shifts 8am-5pm

Community Health Worker Responsibilities:

• Book appointments in Electronic Health Record, according to providers’ scope of practice.
• Use Microsoft Office to communicate and report information
• Perform patient screening surveys; score questionnaires and alert care team
• Document encounters in electronic health records
• Participate in health fairs, community and outreach events to promote company services
• Maintain logs as directed by supervisors
• Participate in health fairs, tabling, collect information
• Schedule in person interpreters, and transportation, via patient health plans as applicable
• Be highly dependable and independent
• Conduct outreach and enrollment services in available public benefit programs
• Connect patients to internal resources and/or services to meet their needs
• Serve as a liaison between the community and health and social services.
• Interface well with clinic staff, community and care teams.
• Meet departmental review and documentation standards for work assignment
• Adhere to the policy and procedures of assigned clinic(s)
• Build and maintain appropriate relationships on behalf of the company
• Support a positive image of the company inside and outside the organization.
• Work flexible hours, which includes some evenings and weekends
• Work at any of the locations, including clinics, mobiles, schools, homeless encampments, warming centers and patients’ homes, or patient appointments as assigned.

Community Health Worker Job Qualifications:

• High School Diploma or equivalent
• Two (2) years of experience in a community-based health care service capacity preferred
• Bilingual preferred
• Proficiency in computer operations, navigational skills and comfortable with Internet-based applications; MS Outlook and Email etiquette
• Phone Etiquette
• CHW Certification - Recertification and/or retraining process, as required
• CPR

Community Health Worker Work Conditions/Physical Requirements:

• Travel to other sites and/or locations required
• Work outside in varying temperatures may be required
• Prolonged periods of standing or sitting.
• Work flexible hours, which includes some evenings and weekends
• Sitting, walking, and standing associated with office environment
• Regular bending, stretching and lifting
• Manual dexterity used to include, but not limited to, standard office equipment, and computer keyboard.
• Traveling to work sites may include exposure to outside elements
• Presenting information in front of a group or individual
• Reading and writing in order to review records for accuracy.
