Covenant House Incorporated, a community-based healthcare organization dedicated to providing high-quality, patient-centered care, is seeking a Temporary Certified Community Health Worker to join our team.
The Certified Community Health Worker will be a vital and trusted member of the healthcare team, serving as a liaison between patients, the community, and health and social service providers. The successful candidate will help individuals navigate the healthcare system, connect with community resources, and promote improved health outcomes through education, advocacy, and care coordination.
Position Responsibilities

Patient Outreach & Engagement
·        Build trusting relationships with patients and families.
·        Conduct outreach by phone, in the community, and during home visits as appropriate.
·        Assist patients in navigating healthcare and social service systems.
·        Re-engage inactive patients into healthcare regimens and promote preventive care participation
Â
Assessment & Care Coordination
·        Complete patient intake screenings and assessments.
·        Identify barriers affecting health outcomes.
·        Support the development of patient-centered healthcare action plans.
·        Coordinate services with healthcare providers and interdisciplinary care teams.

Resource Identification and Navigation: Connect patients with daily living resources that include the following:
·        Food assistance
·        Housing support
·        Transportation
·        Insurance enrollment
·        Behavioral health services
·        Utility assistance
·        Childcare

Community Support and Education
·        Provide education on healthy lifestyle behaviors and chronic disease self-management.
·        Promote preventive screenings and routine healthcare visits.
·        Support patient goal setting and self-advocacy.
·        Documentation & Compliance
·        Document all encounters, referrals, interventions, and outcomes in the electronic health record (EHR).
·        Maintain HIPAA compliance and patient confidentiality.
Skills
·        Participate in quality improvement and program evaluation activities.
·        Demonstrated experience working with diverse communities
·        Excellent communication and interpersonal skills
·        Strong organizational and time management abilities
·        Computer literate
·        Ability to travel throughout Philadelphia and the surrounding communities
Education/Experience Requirements
·        High School Diploma/Equivalent, Associate’s Degree in Public Health, Human Services, Social Work, Healthcare, or related field preferred
·        Community Health Worker Certification is necessary
·        Experience in healthcare, case management, outreach, care coordination, social services, or community-based programs.
·        Knowledge of Philadelphia community resources and social service systems
·        Bilingual in English and Spanish is a plus
