Job Description

The Public Health, Policy and Planning Division of San Mateo County Health is seeking a well-qualified individual for the position of STI/HIV Social Worker III-Spanish Speaking Preferred to provide a variety of services including on-going clinic-based case management and short term STI/HIV risk reduction counseling, and other duties as assigned.

There is currently one limited term vacancy in the Public Health, Policy and Planning Division located in San Mateo, CA, but will travel/drive throughout the County to see clients, attend meetings, case conferences, etc. The schedule is Monday through Friday, eight hours daily, with a flexible schedule between 7 a.m. and 8 p.m.

Primary responsibilities include:
Meeting with clients at least twice a year to determine eligibility for program; intake process for all new clients; program enrollment; enrollment and registration into county, state and federal programs; psycho-social assessment; provide on-going support as needed; develop linkages for clients to adjunct services (MHA emergency housing and financial services; ACRC food vouchers; HRTC psychotherapy services; Medical-Psychiatry; Dental Services, Psychiatry; benefit services, and other resources as needed); one-to-one support (up to and including, medication adherence, wellness health promotion, appointment follow up, assist with travel to appointments, syringe exchange services, sexual and substance use risk/harm reduction counseling, etc.); track clients; and periodic check ins with patients who have fallen out of care.

Additionally, the SW-III will write reports or document work time spent with client in electronic medical records, statewide database, and other data keeping systems; meet with collaborators, and any other meetings with persons related to the program or on behalf of the client; attend monthly or bi-monthly provider case conferences; participate in specialized care coordination meetings for patients; participate in all mental health meetings (held once to twice monthly); participate in monthly program team meetings; participate in monthly providers meeting; and participate in quarterly all staff meetings.

Ideal candidate will have experience in:

  • Providing case management services to HIV+ clients;
  • Professional, personal and/or volunteer experience in HIV care and prevention programs with diverse populations;
  • Risk/harm reduction counseling/therapy;
  • Working as part of an integrated treatment team; and
  • AIDS Drug Assistance Program (ADAP) and Office of AIDS Health Insurance Premium Payment (OA-HIPP).
  • Communicating effectively verbally and in writing.
  • Fluency in both English and Spanish is preferred, but not required.


This is a limited term, at-will position. Limited Term Employees are assigned to assignments not to exceed three years. They receive medical and dental benefits, similar to regular employees, and a defined contribution retirement plan.

The eligible list generated from this recruitment may be used to fill future extra-help, term, unclassified, and regular classified vacancies.

 

QUALIFICATIONS

Knowledge of:

  • Theories of human behavior and their practical applications.
  • Psycho-social, medical, and economic interrelationships that result in individual and social abnormalities.
  • Philosophy and function of social work systems.
  • Appropriate utilization of a variety of social resources.
  • Laws, rules and regulations governing the operation of social work agencies, specifically those relating to the specific areas of assignment.


Skill/Ability to:

  • Formulating, developing and implementing treatment plans.
  • Working effectively with persons differing in value systems, ethnicity, cultural backgrounds and psychopathologies.
  • Working with a variety of social resources to plan and coordinate services to individual clients.
  • Acting as a consultant and trainer to other social service staff.
  • Preparing clear and concise reports.


Licensure/Certification:

  • California class C driver license or equivalent.


Education: Satisfactory completion of 36 semester units of Social Welfare and/or appropriate behavioral science courses.

Experience: Three years experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.

Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.
