Job Description

POSITION SUMMARY

Faculty and Residents from the University of Kansas School of Medicine--Wichita’s Internal Medicine Department provide patient care through our clinic. We are dedicated to continuous quality improvement while providing high quality, efficient health care to patients. As a LCSCW you will provide non-medical interventions for patients as referred by clinic staff and physicians including but not limited to: psychosocial assessment and evaluation, individual cognitive behavioral and solution focused therapy, group therapy, informal case management, crisis intervention, community resource referral and assistance, patient assistance program application and other assistance with access to medical needs, long term care need and advance directive counseling and assistance, insurance counseling and referral, and patient education. Coordinate with clinic director(s) and staff to ensure patient needs are met.

Transplant Program: This position will function as part of a multi-disciplinary team. Primary role is to collaborate,

communicate and facilitate coordination of services post-hospitalization as established by the healthcare team and executed by the case manager. Responsible for the psychosocial assessment of patients, for post hospital discharge needs including home care, nursing home placement, durable medical equipment, financial assistance, counseling, and other community resources. Ensure appropriate decision makers are informed of and involved in treatment planning. Provide support and counseling for patients and family members.

ESSENTIAL FUNCTIONS

Patient Therapy and Case Management (75%)

  • Completes patient assessments (initial and follow-up), develops individualized care plans with each patient, and provides recommendations to physicians for management of patients’ mental health needs
  • Documents assessment data, educational plan, educational services provided and evaluation results in each patient's electronic medical record
  • Provides depression and cognitive screenings, crises intervention, advanced care planning assistance as well as long term care planning counseling services
  • Provides therapeutic services as needed, including but not limited to: CBT, solution focused, and psychotherapy services, as appropriate for short-term patient need
  • Coordinates referrals to outside providers as appropriate for long term therapeutic need for severe and persistent mental illness
  • Assists patients in understanding their diagnosis and medical needs, following up on patient/family needs and communicating with the physician as appropriate
  • Participates in weekly interprofessional meeting to discuss clinic’s Complex Patient Panel and notifies interprofessional team of details regarding coordination and management efforts for complex care patients
  • Provides medication cost assistance and insurance counseling
  • Assists in identifying and completing applications for pharmaceutical assistance programs
  • Identifies community resources for patients, provide referrals as appropriate, and maintain repository of information for clinic staff and providers
  • Participates in clinic process improvement activities and team meetings to ensure efficient operations
  • Attends applicable educational meetings as required to maintain licensure/certifications
  • Other duties as assigned

Special Programs (25%)

  • Provides support for the KU School of Medicine, Kidney Transplant Team in Wichita
    1. Meets with transplant recipient and completes psychosocial evaluation to determine candidacy or donation suitability for transplant and identifies any areas of concern
    2. Meets with patient to discuss post-transplant plans and implication of Transplant on disability, insurance, and rehabilitation
    3. Assists with ongoing transplant needs for the life of the transplant patient
  • Assists with development of special programs such as DPP, DSME, IBT, Weight Loss, etc., providing behavioral health support and services
  • Co-facilitates (with other clinic providers) patient support groups and educational efforts
  • Assists with completion and submission of special program reporting as required

POSITION QUALIFICATIONS

Certificates & Licenses: Licensed in the State of Kansas, LSCSW Certification

Education: Master’s Level in Social Work

Experience:

  • Two to three years related experience
  • EHR experience preferred (we use eClinicalWorks)
  • Microsoft Office experience a plus
