Community Support Specialist

Services for Independent Living seeks a well-qualified, dynamic Community Support Specialist. This person will provide a wide range of public health services and supports, including, assistance with accessing vaccines and connecting to other services, transition, and diversion from high-risk congregate settings to community living, health and wellness programs, activities that address social isolation and social determinants of health, and other activities that support the public health and well being of people with disabilities. Additional responsibilities include providing information and referral and waiver application services as part of the Ohio Benefits Long Term Services & Supports and providing marketing and outreach for assigned grant projects. Applicant must have personal or professional experience with disabilities and be able to address barriers related to disability issues. Ideal applicant will have systems advocacy and community education experience. Excellent organizational and communication skills a MUST. Computer skills necessary. MUST have reliable transportation and meet agency minimum insurance requirements. Applicant must be able to pass background screening. Individuals with a disability strongly encouraged to apply.
