Job Description

Job Summary:

The duties and responsibilities of the LVN include supervising direct care staff, performing direct care according to current standards of practice, state and federal regulations, and facility policy while overseeing the health and wellness of assigned residents. Communicating professionally and effectively with all members of the care team, including residents and families. Works under RN direction.


Job Qualifications:

Education:LVN degree from an accredited nursing programLicensure:LVN active license to practice in the state of TexasExperience:Minimum of one year in nursing experience in long-term or other health care setting. 2+ preferred.Skills:

Assessment and clinical skills (med surg level), supervisory and communication skills, excellent customer service skills.

Basic computer skills.

Current BLS certification.

Certified Dementia Practitioner Certification preferred - the course is offered and required once hired

Transportation:Ability to ensure timely arrival and stable transportation.Exemption Status:Non-Exempt

Environmental and Working Conditions:

The LVN works in a healthcare community environment and must be able to move throughout the buildings and grounds as needed throughout the day. It is possible that the work may include exposure to residents and/or patients with AIDS, HIV, and hepatitis B viruses involving Risk Classification Categories 1, 2, and 3**. Environmental conditions will fluctuate. To be qualified for the position, applicants and teammates will have the ability to work in an environment conducive to caring for residents, with or without reasonable accommodation and without posing a direct threat to themselves or others.

  1. Tasks may involve exposure to blood or body fluids.
  2. Tasks do not involve contact with blood or body fluids but could result in performing a Category 1 task.
  3. Tasks do not involve any risk of exposure to blood and body fluids.

Physical and Mental Effort:

The LVN works in a healthcare community environment and must be able to move throughout the buildings and grounds as needed throughout the day. Ability to lift up to 40 pounds. Requires working under some stressful conditions to meet deadlines.


Essential Functions:

Performs focused assessments, interim assessments for residents within scope of practice.

Assesses/Intervenes/Reports changes in residents' physical/emotional conditions, reactions to medications and treatments, attends and participates in care conferences, and reports to responsible party or physician as needed.

Assesses/ monitors assigned residents' progress and gives feedback on individual interventions with care team.

Performs direct resident care requiring nursing skills (as approved by TBNE and within scope of practice); including, but not limited to IV, Wound Vacs, Injections, Gastrostomy Feedings, Ostomy Care, Tracheostomy Care, surgical drains.

Carries out/coordinates physician orders in a timely manner.

Participates and completes discharge-focused assessments and required patient/family teaching follow-up.

Directs/Assigns work for direct caregivers (CNA, CMA) to ensure quality care for residents.

Communicates clearly and professionally with Families, Residents, Professional Providers, and Teammates.

Requisitions approved materials, supplies, and equipment to carry out planned/ordered care; which may include medication refills being maintained in a timely manner, checking orders for errors and accuracy.

Maintains a safe environment for residents; including adherence to infection control policies.

Participates in ongoing training and in-services for improving knowledge and updating skills.

Performs other assigned tasks as requested and in accordance with policies and procedures.

Establish and encourage an atmosphere of optimism, warmth, and interest in residents' activity preferences and needs.
