Job Description
QUALITY ASSURANCE:
Participates in ongoing evaluation and performance improvement activities.
Participates in devising, reviewing and revising policies, procedures and protocols.
QUALIFICATIONS:
High school diploma required.
Multilingual in English, Spanish, Cape Verdean Creole, Portuguese and/or Haitian Creole essential.
Experience as a CHW or in another position with related duties
Complete the CHW training at CHEC in Boston
Must have a current driver’s license and reliable transportation
MINIMAL KNOWLEDGE BASE:
Knowledge of client advocacy and local social service agencies.
Requires the ability to work with a wide range of professionals, including a multi-disciplinary medical team.
Computer skills, including use of excel
REPORTS TO: Clinical Projects Manager
WORKING CONDITIONS:
A busy community health center located in a downtown business district. CHW will also work in community spaces such as church’s, housing sites and outdoor market sites regularly. Occasional long hours, nights or weekend events may be required.
PHYSICAL REQUIREMENTS:
Physical demands requiring lifting of small pieces of equipment and/or boxes not to exceed 20 pounds. Mostly sedentary and interoffice walking required. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversations with or without audio devices.

POSITION SUMMARY:

PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Works within the Nursing Hypertension team to coordinate services for patients experiencing hypertension or uncontrolled hypertension.
- Works within the Hypertension remote monitoring team to coordinate services, ensure compliance, provide hypertension education, and coordinate Blood Pressure cuff return.
- Provides outreach, health education groups and other services, which expand health equity as assigned by Supervisor.
- Informs providers and clinic staff of services.
- Participate in outreach activities
- Participate in community and outreach activities that identify, recruit and/or educate the patient population.
- Maintains confidentiality at all times.
- Is pleasant, courteous, and considerate of patients, residents, co-workers and partners.
- Represents the Heath Center in a professional manner at all times.
- Attends all required and pertinent meetings.
- Completes and charts outreach and program information, keeps logs up to date at all times.
- Assist patients in addressing barriers to care.
- Attends Individual supervision and CHW group supervision as scheduled.
- Provides numbers and documentation of activities to supervisor.
- Works with supervisor in completing grant reports.
- Performs projects and other duties as assigned by supervisor.

In order to prevent a direct threat to the safety and health of our patients and staff, effective August 1, 2021, new hires are required to have had at least the first dose of a COVID vaccine prior to their start, with the exception of those who have documented medical or religious exemptions. New hires who are not yet vaccinated can arrange to have
the vaccine on site.
