Liberation Programs is one of Fairfield County's leading behavioral health organizations specializing in treatment for all types of substance use disorders including alcohol, opiates, depressants and stimulants. Our goal is to help individuals and families overcome addiction in order to restore their lives and ultimately strengthen our communities. Liberation provides services for youth, adults and families that include two inpatient treatment programs, outpatient and intensive outpatient services, health education for older adults and people living with HIV/AIDS and other chronic illnesses, treatment and resources for adolescents and their families, education and prevention efforts in the community, and permanent supportive housing for families. In operation since 1971, the agency has sites in Greenwich, Stamford, Norwalk and Bridgeport and served more than 2,400 individuals in our last fiscal year.

Job Description:

The Engagement Specialist is responsible for assuring expedient admissions of persons seeking treatment, conducting assessments, assigning the person to the appropriate level of care and ensuring that programs are at capacity. The Engagement Specialist maintains and manages a waiting list, coordinates all referrals received, conducts outreach toward engaging referrals to treatment, and assures the recoveree receives a full orientation to treatment.

1. Coordinate and manage admissions processes

  • Manage walk-ins and calls from persons seeking treatment
  • Manage referrals from key sources to assure expedited intake
  • Screen applicants and schedule intake date.
  • Prioritize admissions and manage waiting lists
  • Assists applicants not admitted with a referral to an alternate setting and documents reasons for not admitting persons into treatment

2. Manages recoveree admissions in a manner that creates a welcoming environment and fosters his/her engagement in treatment.

  • Assure completion of financial information and pre-authorization.
  • Complete initial assessment of applicant and determination of appropriate level of care.
  • Identify any crisis issues to be addressed within first 3 days of treatment and reviews these needs with the respective treatment teams.
  • Coordinate with physician and medical assistant for medical screening in order to assure all medical screens are completed in a timely manner.
  • Coordinate counselor assignment with Program Director.
  • Follow up with recoveree within 10 days of admission to assure completion of orientation to treatment.

3. Networks with and conducts outreach to referral resources.

  • Represents Liberation Programs at local community meetings; advances LPI as a resource in the community.
  • Establishes and maintains collegial and collaborative relationships with partner services and state and local agencies and facilitates referrals to LPI.
  • Reports to Program Director meetings attended, opportunities for partnership development or network enhancement identified and referrals that emerge from participation in community meetings.

4. Manages census and utilization data and provides reports as needed.

  • Reports daily admissions, bed vacancies, waiting list; assures program is at full capacity.
  • Reports monthly on admissions outcomes data including but not limited to: # of applicants, # screened, # admitted, average length of time to complete admissions process.
  • Reports on discharges
  • Identifies any trends regarding utilization rates.

Other Duties:

  • Under the direction of the CEO or CPO, assist with special projects, as needed.

Experience and Educational Requirements:

  • CT State License preferred (LCSW, LMFT, LADC, LPC)
  • Minimum of two years' experience in behavioral health screening, assessment and referral processes.
  • Familiar with data management and electronic medical records systems.
  • Strong organizational skills and abilities to handle multiple priorities and meet deadlines while providing exceptional customer service.
  • Knowledge of state and federal behavioral health regulations preferred.
  • Excellent computer skills
