RN Case Manager Hospice

RN Case Manager position: Management and/or Hospital Administration experience a plus!

The Hospice RN Case Manager oversees hospice care, maintains current plan of care and works as a leader in the Hospice IDG team to ensure patient and family needs are met at end of life. Visit patients in the Pittsburgh area to provide a nursing assessment, coordinate with facility staff, work with physicians to obtain orders and communicate with compassion to families. Position is salaried, plus paid mileage and cell phone stipend.  Hours are Monday - Friday, 8am - 5pm but we are willing to talk with you to discuss alternative shifts if desired.

The opportunity

We are seeking a caring RN Case Manager to join the Bethany team.  

Requirements:

• RN License in PA
• At least one year of nursing experience, hospice experience, management and/or hospital administration is a plus
• Vehicle, driver’s license and car insurance
• Understanding of hospice philosophy and needs of the terminally ill (we can help you with this one!)
• Satisfactory criminal background check
• Presentation of negative TB test or CXR as required by state law
• Demonstrate good nursing judgement and excellent observation and communication skills 
• Ability to provide good documentation in a timely manner
• Desire to be part of the best hospice team in western Pennsylvania
