POSITION SUMMARY:
The Hospice and Palliative Liaison builds relationships within the communities that we serve in order to promote the services provided and ensure that we are meeting the needs of our patients, families, and partnering facilities. The community
Liaison personnel pursues new referral sources and ensures continued service among existing accounts (i.e., physicians, facilities, hospitals, social workers, and community organizations, such as church groups and senior centers). This position is responsible for representing Peaceful Hope Hospice and Palliative Care, LLC. in the community, event planning, and marketing communications (including email, online, social media, and direct mail campaigns).

ESSENTIAL JOB FUNCTIONS:

1. Reaches out to referral sources to assure the agency is meeting their needs, as well as those of the patients and families.

2. Develops and maintains contact with key community and consumer organizations to provide hospice and palliative education and updates of Peaceful Hope Hospice and Palliative Care services.

3. Makes calls to potential referral sources each day to promote hospice services and arrange meetings.
4. Effectively communicates with the operational, intake and clinical team regarding new patient referrals and receives updates regarding referred patients.
5. Coordinates with clinical management in planning in-services and education to the healthcare community.
6. Represents Peaceful Hope Hospice and Palliative Care positively and professionally within the healthcare community.
7. Collaborates with the Director of Business Development to execute professional events and engagement opportunities.
8. Assesses and provides quarterly reports to the Director of Business Development regarding marketing trends.
9. Provides suggestions for marketing tools and collateral to enhance the positioning and branding of the agency.
10. Develops and executes quarterly marketing plans.
11. Identifies areas of opportunities for improvement that will assist the company in ensuring the customer needs are being met.
12. Serves as a liaison between the Hospice Medical Director and primary care physicians.
13. Initiates, completes, and submits all sign-up paperwork to the Peaceful Hope Hospice and Palliative Care office within 24-hours of completed sign up.
14. Obtains data on physical, psychological, social, and spiritual factors that may influence patient/family health status and communicate these needs effectively.
15. Seventy-five (80%) of work week will be spent in the field.
16. Responsible for meeting with patient and families to describe the hospice philosophy and election of hospice benefit.
17. Participates in external committees such as Readmission groups, ACO’s and other healthcare related events.
18. Organizes Hospice Education for other healthcare providers, providing continuing education for those disciplines which the agency is regulated to do so.
19. Participates in annual competitive and program analysis.
20. Receives approval from the Director of Business Development prior to expense spending.
21. Facilitates educational presentations to both small and large groups in the communities Apex serves.
22. Responsible for meeting admission goals set forth each quarter by the agency.
23. Responsible for census growth of the agency.
24. Responsible for reporting complaints in accordance with agency policy.
25. Serves on internal agency committees as assigned.
26. Presentation of various services to include the Legacy Program, volunteer, and specialty services.
27. Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state, and organizational policies.
28. Other duties as assigned.
REQUIRED SKILLS, ABILITIES AND KNOWLEDGE:

1. Ability to work independently, while collaborating with managers and other personnel.
2. Ability to evaluate a situation quickly and respond appropriately.

5. Ability to utilize electronic medical records and other computer programs.
6. Works with diverse social and economic situations without imposing own values on patient or family.

9. Demonstrates understanding of HIPAA Privacy Standards and related agency policies.
10. Willingness to work a flexible schedule in order to serve patients and families 24-hours a day who require hospice services.
11. Must be able to make presentations to small and large groups.
12. Knowledgeable in the Anti-Kickback Statutes and Stark Laws.

QUALIFICATIONS:

1. Minimal of an Associate Degree in Business, Marketing or Communication; or equivalent work experience in a sales environment.
2. Minimum of two (2) years- experience in a Marketing/Sales environment; hospice or home health setting preferred.
3. Computer proficiency in Microsoft, Excel and PowerPoint.
4.Experience in public speaking with demonstrated presentation skills.
5.Must be a licensed driver with an automobile in good working condition and insured in accordance with state and organizational requirements.
6. Complies with accepted professional standards and practice.
7. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

Job Type: Full-time

Pay: From $110,000.00 per year

COVID-19 considerations:
All employees are required to wear mask when out in the field in addition to social distancing when necessary and hand hygiene. Management continuously communicates with all employees regarding changes or COVID related restrictions in the community.

  • Hospice care: 2 year (Preferred)
