What You'll Do: As a Hospice RN, you will cover the Stamford and Norwalk CT areas and your mission will be to deliver compassionate and personalized patient care in alignment with physician's orders. Your responsibilities will include:

Regularly assesses and reassesses the nursing needs of the Hospice patient. Competently implements care needed for comfort and palliation of symptoms
Provides direct client care and works closely with case manager in implementing care.
Coordinates the implementation of the plan of care for patients residing in SNF, ALF, hospital contract bed or personal residence.
Provides dietary counseling.
Initiates nursing procedures appropriate for the patients Hospice care and safety.
Teaches, supervises and counsels the Hospice patient, family members and facility staff about providing care for the client.

Experience:

Graduation from an accredited school of nursing; BSN preferred
Currently licensed to practice as a Registered Nurse in the State of Connecticut.
A minimum of two years of nursing experience, preferably in medical/surgical, oncology , Critical Care, ED or labor and delivery nursing. Home Health Care and/or Hospice experience preferred. Active patient contact within past three years, preferred.
Ability to work independently, make accurate, and at times, quick judgements.
Able to drive in all types of weather.
