Job Description: Hospice Case Manager
Position Overview:
The Hospice Case Manager is responsible for coordinating and overseeing the care of hospice patients and their families. They serve as a primary point of contact, providing comprehensive support and guidance throughout the hospice care process.
Primary Responsibilities:
- Conduct initial and ongoing assessments to determine patients' hospice needs and develop individualized care plans.
- Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and spiritual counselors, to deliver holistic care.
- Provide skilled nursing care, including medication management, symptom control, and wound care, as needed.
- Coordinate services and resources to meet patients' physical, emotional, and spiritual needs, ensuring continuity of care.
- Offer emotional support and counseling to patients and their families, addressing their concerns and facilitating difficult conversations.
- Educate patients and families on hospice philosophy, treatment options, and end-of-life care planning.
- Document patient assessments, care plans, and interventions accurately and timely in accordance with regulatory standards.
- Communicate effectively with healthcare providers, caregivers, and community resources to optimize patient outcomes.
- Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives.
- Adhere to ethical and legal standards, maintaining patient confidentiality and respecting patients' autonomy and dignity.
Qualifications:
- Bachelor's degree in nursing (BSN) from an accredited institution (preferred).
- Current registered nurse (RN) licensure in the state of practice.
- Minimum of 1-2 years of clinical experience in hospice, palliative care, or a related healthcare setting.
- Strong assessment, communication, and critical thinking skills.
- Ability to work independently and collaboratively in a fast-paced, interdisciplinary environment.
- Compassionate demeanor and a commitment to providing high-quality, patient-centered care.
- Proficiency in electronic medical record (EMR) documentation and computer skills.
- Valid driver's license and reliable transportation for home visits (if applicable).
