ASSISTANT DIRECTOR OF NURSING


Job Summary:


The Assistant Director of Nursing will oversee and direct all aspects of patient care for all home care
patients on a day-to-day basis. The ADON will ensure the delivery of quality patient care through
professional assessment, intervention, evaluation and supervision of the entire patient care team and
plan of care in collaboration with the Director of Nursing.

Qualifications:
1. A minimum of 3 years nursing experience, hospice preferred.
2. Previous management experience.
3. Current and unrestricted nursing license; LPN or RN (RN preferred)
4. Ability to work well within an interdisciplinary team setting as well as independently.
5. Demonstrates the ability to assist with and respond to the needs of terminally ill patients.
6. Ability to communicate well with others both written and orally.
7. Ability to work a flexible schedule.


Responsibilities:
To ensure that all clinical services provided are consistent with the hospice standards of care and comply
with all written policies and procedures.
- Coordinates and supervises the overall provision of home care for hospice patients and their
families.
- Ensures that clinical records are maintained in compliance with state, federal and agency standards.
- Assist in the admission process as appropriate as serves as a primary clinical point of contact to the
Clinical Administrative Assistant
- Serve as a consultant to each team member by providing clinical expertise and assistance as needed.
- Facilitates documentation as needed for certification, re-certification and/or patient
discharge/revocation within the EMR and manages expired patient process.
- Supports ongoing patient care needs by performing visits routinely to include but not limited to
admissions, re-visits, and pronouncements.
- Performs information visits and collects patient data for evaluation of new admissions, completes
consents & legals as appropriate.
- Assists in providing administrative support in coordinating on call nursing schedule.
- Provide written evaluations of appropriate staff as necessary.
- Monitor and adjust the caseloads of each clinical staff member and oversee the schedule of weekly
visits in conjunction with the DON.
- Facilitates healthcare & national recognition activities for patients at the direction of the DON.
- Support volunteer activities as needed/by request.
- Coordinates and manages all DME related activities & patient coordination.
- Updates and manages the physician & facility profiles within database in EMR.
- Assist in orientation & training for all AIDEs and LPNs, supports RN onboarding in conjunction with
DON.
- Coordinates support from each IDG member as appropriate.
- Manages IDG notebook for compliance (i.e. IDG sign in log, print out of IDG summary/overview
page, includes copy of any education materials presented)
- Ensures policy and procedures are followed.
- Ensures effective communication for staff, patients, and families.
- Assure that standards of ethical business and clinical practice are maintained.
- Tracks and records fall monthly for DON review, ensures incident reports are printed for notebook
accordingly.
- Monitors and ensures compliance with Federal and State regulatory agencies and other certifying
bodies.
- Reports grievances and complaints made by patients or families to the appropriate persons.
- Reports all allegations of patient abuse and/or misappropriation of patient property.
- Reports all accidents and incidents. Identifies and responds appropriately to emergency situations.
- Observes and ensures safety needs of the patients.
- Assist in the marketing of the hospice by highlighting the company as a community resource.
- Acts as the primary point of contact for ALFs, PCHs, LTACs and makes rounds routinely to maintain
relationships.
- Perform other duties as assigned by the DON & Compliance Officer

ability to adjust focus.
The noise level in the work environment is usually moderate.
Exposure to disagreeable conditions including odors, infection, illness, and physical contact from
disruptive patients.


Mental Requirements:
Must be able to work independently, must be flexible, innovative and possess good interpersonal skills.
Must be able to cope with mental and emotional stress and demonstrate emotional stability.


Working conditions:
Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and
excrements, adverse environmental conditions & hazardous materials.


Transportation:
Must have a current valid driver’s license, auto liability insurance and reliable transportation
