The Position

This is a Limited Duration position. Persons appointed to limited term positions will be designated as limited duration employees whose appointment shall not exceed two (2) years except for the extension of grants or funding from outside sources.

The Registered Nurse (RN) will be part of a multidisciplinary team supporting advanced health care delivery modalities in the pre-hospital care setting for Portland Fire & Rescue (PF&R). The RN will work under the direct supervision of the Community Health Critical Care Nurse Manager (CHNM) and will be responsible for shift supervision and healthcare advisement of the Community Health Assess and Treat Teams (CHAT) in the field. The RN is responsible for managing EMT Basic's and/or Paramedic's in the delivery of health care to community members within PF&R's jurisdiction of authority. The RN will work under the direction of Multnomah County's Emergency Medical Director and is responsible for advising all direct patient care delivered by PF&R EMT Basics/Paramedics in accordance with the guidelines and protocols established by the Medical Director and PF&R.

GENERAL ACCOUNTABILITIES AND ESSENTIAL FUNCTIONS 1. CLINICAL QUALITY: The RN will perform to the highest standards of nursing. The following responsibilities also apply:

+ Advise CHAT teams in regard to physical assessments and charting of observations of patient's condition at each visit.

+ Complete evaluation tasks, including reviewing medication and vital signs when requested by CHAT teams.

+ Administer Physician-prescribed medication, when outside the scope or protocols of the CHAT teams. + Dress or redress wounds and assess the healing progress, as requested by patient's physician or CHAT team's referral. + Utilizes critical thinking skills in the nursing process: assist EMT's/Paramedics in assessing conditions, demonstrating effective decision making, implementing appropriate interventions, anticipating potential complications, and evaluating patient outcomes. + Responsible for coordinating Patient Education. + Interprets lab values and directs care as appropriate. + Prepares clinical and progress notes for each patient visit and summaries of care conferences with patients and/or family in a timely manner. + Counsels the patient and family in meeting nursing and related needs. + Participates in and presents in-service programs. + Follows Medical Director's protocol guidelines and notifies physician of patient needs and changes in condition. + Refers to appropriate multidisciplinary services (Social Work, Therapists, and Treatment Facilities) as appropriate. + Conducts patient care conferences when appropriate. + Available to CHAT Teams or patients for referral and/or follow up services as needed. + Facilitates teamwork across all care settings.

2. MANAGEMENT- The RN will report to the Community Health Critical Care Nurse Manager. The following responsibilities also apply:

+ Communicate program standards with CHAT team members. + Keep organized records of clinical documentation. + Provide training to EMT's, Paramedics, and other health professionals.

. 3. SAFETY: The RN will conduct all work in a safe manner in accordance with PF&R policies. The RN will maintain the safety of all customers throughout the provision of quality care and services.

+ Adheres to adequate staffing recommendations set forth by PF&R. + Ensures the safe operation of all company vehicles with respect to patient safety. + Enforces federal/state/city rules and regulations and bureau safety policies.

4. BUSINESS DEVELOPMENT: The RN is responsible for assisting the Community Health Assess and Treat teams (CHAT) in the prehospital care setting .

+ Outcome measurement and reporting. + Evaluates community health disparities and provides focused health education outreach to those communities. + Evaluates Equity components related to Quality metrics set for by the State of Oregon. + Monitors high utilizers of emergency medical services and develops sustainable programs to reduce unnecessary 911 calls. + Engages in regular progress reporting to reassess program goals and required elements for sustainability. + Responds to customers and co-workers in a timely and professional manner.
