Medical Case Manager, MSW

Under the clinical supervision of the Program Director, and in collaboration with the Medical Care Team,and the Ryan White Medical Case Management team, this position is specifically responsible for providing direct aging clients medical case management support for Ryan White patients. Services include linkage to Psychiatric, counseling and support group management for psychosocial support, HIV/AIDS psycho-education, ADAP, Medical/Medicare dual RW>50 program enrollment, referral to dental, optical, mental health, housing, legal services and other services to meet the hands on further support beyond the basic needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Provide Individual in person for homebound and aging clients weekly, monthly psychosocial and case management support.
• Conducts semi-annual case management assessment to clients to determine needs and develop care plans
• Provide referrals for substance use/abuse services, housing legal, mental health, primary care, dental, and optical services.
• Promotes adherence with medical visits and with medication, coordinates with pharmacies to assist patients as needed.
• Documents patient contacts and communicates with the medical care team using the patient’s electronic health records.
• Attends monthly HIV targeted aging client services meetings with manager, case conferences, QI meetings, and other meetings as assigned.
• Meets weekly with MCM supervisor for clinical aging services client support
• Maintains all HIPAA standards of privacy and confidentiality
• Other duties as assigned.

EXPERIENCE AND TRAINING:

• Licensed RN, Physicians Assistant, LVN, LCSW or Masters degree in social work, human services, Public Health or Masters degree candidate in combination with education and relevant experience will be considered.
• Knowledge of HIV/AIDS clinical and supportive priorities, HIV further progression prevention and case management and referral methods.
• Knowledge of, and respect for a variety of Santa Clara County, African American, Latino communities and other populations of high risk for gaps in aging HIV services support
• Must be committed to supporting clients toward the goals of active participation in Primary Care as well as commitment to services based and focused on wellness and recovery.
• Minimum of two years professional social work experience working as part of an interdisciplinary team in a healthcare setting with an emphasis on chronically ill patients preferred.
• Bilingual in one or more threshold languages (Spanish, Vietnamese) preferred.
• Knowledge of HIV/AIDS. Adept at establishing rapport with clients, families, health care professionals, and community service organizations.
• Reliable transportation for daily use, a valid California driver’s license, and liability insurance as required by the State of California
