Care Coordinator

Senior Pro Services, LLC a CHAP accredited Home Health Care Agency is seeking to hire a Care Coordinator to act as an patient intake and to also be in charge of coordination of services and communication between patients, family members, as well as caregivers and other providers.

The duties also include scheduling between patients and their health care service providers. Also to coordinate the admission process for new and returning patients. Individual will assist with the business marketing team. Reports to DON (Director of Nursing), as well as to the Clinical Nurse Supervisor and the CEO.

Who You Are:

With a high school Diploma or GED required. Much preferable for individuals who are a graduate from a junior college or higher educational institution with a degree. Some knowledge or coursework in healthcare is a plus.

Strong communication and computer skills with proficiency in Microsoft Applications (Office) as well as other software is essential. Must possess strong interpersonal and leadership skills to be able to supervise caregivers and to be able to work with other staff.

Job Type: Full-time (Mon – Fri). Some weekends and holidays with OT pay. Participation in paid rotational after hour on-call services is required.

Pay: $28.00 / Hr. and up depending on experience and educational level.
