RN Case Manager Opportunity Description

We are currently seeking RN Case Managers to manage cases and clinical teams in the greater Atlanta, GA and surrounding areas. Home Health experience is preferred but other post-acute case management may be considered. Worker’s Comp experience is a plus. RN Case Manager Job Responsibilities

• Case management that includes 50% of the time in the office, and 50% of the time in the field.
• Oversee clinical team of LPN’s and C.N.A’s as well as participating in hands-on patient care.
• Ensure quality care, and compliance with all state and federal regulations.
• Perform initial client assessments
• Oversee admissions, re-certifications, and discharges of our home care clients
• Coordinate the appropriate care plan for clients
• Work collaboratively with other healthcare disciplines

RN Case Manager Job Requirements

• Post-Acute Clinical Case Management experience as a Registered Nurse required. Home Health and / or Worker’s Comp experience preferred.
• Strong clinical skills: assessments, IV’s, burns, trach’s and vents, mobility transfers etc. Supervisory experience a plus.
• Reliable transportation and the ability to work a vast territory. Mileage reimbursement included.
