Community Health & Outreach Coordinator

POSITION SUMMARY
Reporting to the Chief Community Programs & Outreach Officer, the Community Health & Outreach Coordinator will coordinate events in the community, assist with 340b, support the Mobile RV outreach, schedule events on the calendar of participants, send invites, connect with patients, and send reminders of community events. Support Quality and Marketing efforts.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Manage the 340B clinical inventory and reconciliation
- 340B internal Audits, including chart review
- Outreach/Grants/340 Meeting minutes
- Manage the Mobile RV Schedule ensuring successful planning and execution of initiatives.
- Support referral and partnership resources within the community, schedule meetings, and assist with generating referrals to meet target goals.
- Assist with fundraising communication
- Distribute patient and community-focused Social Media campaigns on behalf of the organization and specific clinics in the assigned service area and other areas of interest.
- Provide outreach support for site-specific clinical and program initiatives.
- Coordinate staff and volunteers to staff patient and community outreach events and initiatives.
- Identify & attend key local and regional meetings.
- Maintain a current resource list on five topics or key community service areas of importance to patient population (i.e. health education and peer support)
- Other duties as assigned.
EDUCATION, TRAINING, AND EXPERIENCE
- 2 Years of Community Health Experience required. (MA, CHW, CNA)
- High School Diploma or equivalent required, some college preferred
- Medical Terminology preferred
- Bilingual preferred (English/Spanish)
- Excellent customer service skills
- Must possess excellent listening and problem-solving skills
- Excellent oral, written, and interpersonal communication skills
- Must be reliable and dependable
- Ability to work with minimum supervision
- Proficient in MS Office Suite
- Proficient in EMR (Centricity a plus)
