Adoration Hospice is seeking a licensed LPN or RN with strong clinical and communication skills to join our team as a Hospice Intake Coordinator in Chattanooga, TN. This is a critical role that helps patients and families transition smoothly into hospice care by coordinating the intake and admission process from start to finish. As an Intake Coordinator You will:
  • Guide patients and families through the hospice intake and admission process with care and clarity.
  • Review referrals and medical records to ensure patients meet hospice eligibility.
  • Triage incoming referrals and coordinate urgent or same-day admissions when needed.
  • Educate patients and families on hospice services, answering questions and easing concerns.
  • Coordinate services, equipment, and care needs with our team and external providers.
  • Verify insurance, obtain authorizations, and document all referral and patient information.
  • Communicate with physicians, discharge planners, and other healthcare professionals to ensure continuity of care.
  • Ensure all regulatory and compliance steps are followed throughout the intake process.
  • Serve as a knowledgeable resource for hospice criteria and support referral sources with alternatives if needed.

Qualifications
  • Registered Nurse or Licensed Practical Nurse, with current and unencumbered state licensure.
  • At least two years of healthcare experience, preferably in hospice operations.
