Swift Health Physical Therapy is seeking a dedicated Full-Time Physical Therapist to join our dynamic team at our rehabilitation facility located in sunny Tampa, Florida. This position is onsite at our state-of-the-art rehabilitation facility, where you will work directly with patients in a collaborative and therapeutic environment. • Conduct comprehensive evaluations of new patients, collecting pertinent clinical history and performing targeted assessments to create tailored treatment plans.
• Effectively communicate assessment results and treatment plans to patients and their families, involving them actively in the goal-setting process.
• Implement individualized treatment protocols based on evaluation findings, adapting to the unique needs and progress of each patient throughout their rehabilitation journey.
• Consult with other healthcare professionals as necessary, documenting any changes in patient conditions that may require updates to their treatment plans.
• Manage discharge planning and follow-up care, ensuring all documentation is thorough, accurate, and reflective of the patient’s treatment and recovery journey.
• Meet productivity expectations through effective time management and maintain an organized schedule for efficient patient care.
• Engage in community outreach and promotional activities to enhance patient access to care and increase awareness about our services.
• Contribute to the department's strategic goals, adhering to all regulatory and accreditation standards.
• Participate in ongoing professional development, seeking opportunities for advanced education and serving as a mentor for students or less experienced colleagues.
• Foster positive patient experiences through adaptability and responsiveness to their needs.

Our patients at Swift Health comprise a diverse demographic with varying rehabilitative needs, including post-surgical recovery, sports injuries, chronic pain management, and neurological conditions. The appointed Physical Therapist will work closely with individuals of all ages, emphasizing a personalized approach that respects their unique circumstances and goals.

• Bachelor's degree in Physical Therapy or a related field.
• A valid Physical Therapy license in the state of Florida is required.
• In-depth understanding of physical therapy principles, practices, and modalities.
• Current certification in Basic Life Support (BLS) for Healthcare Providers is necessary.
• Two years of relevant experience as a Physical Therapist.

• Master's or Doctorate in Physical Therapy (DPT) is highly encouraged.
• Experience in outpatient rehabilitation settings, particularly with diverse patient populations.
• Familiarity with electronic medical records (EMR) systems for documentation and patient management.
