Job description

CareCore Health is seeking an experienced and motivated Registered Nurse to join our team as a Regional Clinical Support RN supporting our skilled nursing facilities throughout the Cleveland and Northern Ohio region.

This role is ideal for a strong clinical leader who enjoys variety, problem-solving, team collaboration, and supporting facility teams to maintain high standards of resident care and regulatory compliance. Travel between facilities throughout Northern Ohio is required.

Responsibilities:

  • Provide clinical support and guidance to nursing teams across multiple skilled nursing facilities

  • Assist with clinical operations, audits, regulatory compliance, and quality improvement initiatives

  • Support facilities with survey readiness and follow-up action plans

  • Collaborate with DONs, ADONs, and facility leadership teams

  • Assist with staff education, mentoring, and clinical coaching

  • Support staffing and clinical coverage needs as necessary

  • Review clinical documentation, care plans, and nursing processes for compliance and accuracy

  • Help drive positive clinical outcomes and continuity of care across the region

  • Travel regularly throughout the Cleveland/Northern Ohio market

Qualifications:

  • Active and unencumbered Ohio RN license required

  • Previous skilled nursing or long-term care experience required

  • Prior leadership experience preferred (DON, ADON, Unit Manager, Clinical Consultant, etc.)

  • Strong knowledge of state and federal nursing home regulations

  • Experience with survey readiness and clinical compliance

  • PointClickCare (PCC) experience preferred

  • Strong communication, organization, and leadership skills

  • Ability to travel throughout Northern Ohio facilities as needed

  • Valid driver’s license and reliable transportation required
